摘要
目的:探讨以医联体为支撑的"1+1+1+X"全科医生签约管理的合理性及有效性。方法:2016年在柯桥区平水镇试行以区镇一体化紧密型医联体为支撑,由专家团队、首席医生、签约医生和责任团队组成的"1+1+1+X"全科医生签约管理团队以中心辖区内常住户籍居民为主,以其中慢性病患者、60岁以上老年人、孕产妇、残疾人等特殊人群为重点,试行家庭医生管理制,观察有效签约率、社区首诊率、双向转诊率、预约门诊率和患者满意率。结果:至2016年底,平水镇全科医生签约服务规范率为25%,重点人群覆盖率为62%;基层首诊比例为62.86%,双向转诊率30%以上,患者满意率99%。结论:签约医生为服务主体,全科团队为依托,区级专家服务体系为支撑,以患者需求为对象,提供连续性医疗服务,可以提高社区居民对签约医生的满意度和信任度,推进双向转诊有序进行。
Objectives： To analyze the rationality and effectiveness of medical alliance depended general practitioner ＇1＋1＋1＋X＇ contract based mode management. Methods： District -town integrated tight conjoint medical alliance support ＇1＋1＋1＋X＇ composed with expert, chief doctor, primary doctor and primary team in Pingshui town, Keqiao district has built since 2016. Key population such as chronic disease, the elderly, the maternal and the disabled received careful service by the team and family doctor management system. The effective contract rate, community first diagnosis rate, dual referral rate and patient satisfaction rate were compared. Results： The effective contract rate is 25%, key population coverage rate is 62%, community first diagnosis rate is 62.86, dual referral rate is 30% and patient satisfaction rate is 99% in 2016. Conclusions： Tight conjoint medical alliance expert support, general practice team based primary doctor responded management mode can provide accessible and sustainable care for the residents and improve patient satisfaction and trust, which can help promoting dual referral development.
