综合呼吸训练对肺癌肺叶切除术患者术后呼吸功能和运动功能的影响 被引量:33

Effect of comprehensive respiratory training on pulmonary function,respiratory function and exercise function in patients with lung cancer after pneumonectomy

摘要 目的探讨综合呼吸训练对肺癌肺叶切除患者术后肺功能、呼吸功能及运动功能的影响。方法选取2014年2月至2016年2月间新疆医科大学第一附属医院收治的68例行肺叶切除术的肺癌患者,按时间先后分为观察组与对照组,每组34例。对照组患者采用常规护理方式,观察组患者在常规护理方式基础上加综合呼吸训练护理,比较两组患者肺功能、呼吸功能及运动功能。结果护理后,观察组患者呼吸频率明显低于对照组,一秒用力呼气容积(FEV1)和最大肺活量(VCmax)值均明显高于对照组,差异均有统计学意义(均P<0.05)。护理前,两组患者Borg评分和6分钟步行试验(6MWT)评定比较,差异无统计学意义(P>0.05);护理后,观察组患者Borg评分明显低于对照组,6MWT评定明显高于对照组,差异均有统计学意义(均P<0.05)。护理后,观察组患者精力指数、体能指数、社会活动指数及健康指数评分均明显高于对照组,住院时间与不良反应情况明显少于对照组,差异均有统计学意义(均P<0.05)。结论综合呼吸训练能促进肺癌患者肺叶切除术后肺功能、呼吸功能以及运动功能的恢复,提高临床治疗疗效,减少患者住院时间及经济负担,并发症少,值得在临床上推广使用。 Objective To investigate the effect of comprehensive respiratory training on pulmonary function,respiratory function and exercise function in patients with lung cancer after pneumonectomy.Methods A total 68 patients undergoing lobectomy for lung cancer at First Affiliated Hospital of Xinjiang Medical University from February 2014 to February 2016 were selected. According to the admission sequence,the patients were divided into an observation group and a control group with 34 patients in each group. The observation group was given comprehensive respiratory training combined with routine nursing and patients in the control group were given routine nursing. The effect of different nursing modes on pulmonary function,respiratory function and exercise function were observed and compared. Results The respiratory rate in the observation group after nursing care was significantly lower than that in the control group( P〈0. 05) and the FEV1 and VCmax value was significantly higher than in the control group( all P〈0. 05). Before nursing care,three was no significant difference in Borg score and 6 MWT between the two groups( P〈0. 05). After nursing care,the Borg score was significantly lower in the observation group than in the control group( P〈0. 05) and the 6 MWT was significantly higher than in the control group( P〈0. 05). After nursing,the scores of energy index,physical index,social activity index and health index were significantly higher in the observation group than those in the control group( all P〈0. 05) and the length of hospitalization was significantly less in the observation group than that in the control group( P〈0. 05). The adverse reactions in the observation group were significantly less than that in the control group( P〈0. 05). Conclusion The comprehensive respiratory training can improve pulmonary function,respiratory function and exercise function,clinical curative efficacy and reduce hospitalization time,economic burden and complications in patients with lung cancer after pneumonectomy. It is worthy of promotion in clinical use.
