微生物检查结果干预临床抗感染治疗的效果观察 被引量:3

摘要 目的:根据病原学微生物培养结果,干预临床抗感染治疗中不合理的抗菌药物,防止细菌耐药。方法:临床药师每日实时监测微生物实验室细菌培养结果,重点关注药敏结果与所用药物不适宜的患者,对多重耐药菌尤其重点关注,科室临床药师随时监测抗菌药物使用后续情况,发现问题及时反馈医师,共同商讨治疗方案。结果:微生物实验室细菌培养阳性结果 479份中,临床药师通过实时监测细菌耐药,根据微物生物培养结果,共干预不适宜的抗菌药物48例,干预率10%,尤其是多重耐药菌的药物选择,不仅防止细菌进一步耐药,促进疾病的治愈,同时也提升了临床药学工作开展的深入性。结论:在细菌耐药监测工作中,临床药师不能仅限于定期分析抗菌药物对细菌的耐药率,更多的是通过实时监测细菌耐药,根据微物生物培养结果,提前干预不适宜的抗菌药物,帮助临床解决实际问题,切实做到防止细菌耐药。 Objective: To prevent bacterial drug resistance by intervention of irrational anti-infection treatment with antibacterial drugs according to the results of etiologic microorganism culture. Methods: The clinical pharmacist monitored the lab cultivation results of microorganism bacteria in realtime daily, especially payed attention to the patients who had drug allergy and for whom unsuitable drugs were prescribed, with focus on multiple drug-resistant bacteria. The clinical pharmacist monitored the subsequent situation after the anti-bacterial drugs were administered, and provided feedback to the doctors of any identified issues for discussion on future treatment plan. Results: Among the 479 positive cases of bacteria culture, 49 cases of unsuitable antibacterial drug treatment were intervened. The intervention of selection of drugs with multiple drug-resistant bacteria not only prevented bacterial drug resistance, promoted the cure of diseases, but also enhanced the depth of clinical pharmacy work. Conclusion: When monitoring bacterial drug resistance, pharmacists shall not limit their work to periodically analyzing drug resistance ratio of baceteria. More importantly, they can intervene in unsuitable antibacterial drugs through real time bacterial drug resistance monitoring according to microbiological culture results, so as to help resolve actual clinical problems and prevent bacterial drug resistance.
