摘要
目的探讨胆道手术胆汁外引流患者经鼻腔肠管胆汁回输对炎性反应以及肠道功能的影响。方法选择河北医科大学附属华北石油管理局总医院于2016年1—2017年5月收治的胆道手术胆汁外引流80例。按是否回输胆汁分为观察组和对照组,每组40例。观察组将胆汁加热净化后,通过鼻腔肠管回输到空肠内;对照组经鼻胃管输注生理盐水。比较两组围术期炎性反应及肠道功能恢复情况,记录并发症及不良反应发生情况。结果观察组术后第5、7 d C反应蛋白、降钙素原均低于对照组(P<0.05)。观察组术后肛门排气时间、肠鸣音恢复时间早于对照组,腹胀、腹泻发生率低于对照组(P<0.05)。结论胆道术后胆汁通过鼻腔肠管回输,可以改善术后炎性反应,促进肠道蠕动功能的恢复。
Objective To investigate effects of bile reinfusion by nasojejunal canal on inflammatory reaction and intestinal tract function in patients after bile external drainage surgery. Methods A total of 80 patients after bile external drainage surgery admitted during January 2016 and May 2017 were divided into observation group( n = 40) and control group( n = 40) according to whether or not having bile reinfusion,Bile reinfusion by nasojejunal canal was performed after being heated and purified in observation group,while physiological saline reinfusion by nasogastric canal was performed in control group. Conditions of inflammatory reaction and intestinal tract function recovery in perioperative period were compared,and incidence rates of complications. and adverse reactions were recorded in two groups. Results Compared with those in control group,in observation group,levels of C-reactive protein( CRP) and procalcitonin( PCT) at the postoperative 5 thand 7 thd were significantly lower( P < 0. 05); postoperative times of gas passage by anus and bowel sound were significantly shorter,while incidence rates of abdominal distension and diarrhea were significantly lower( P <0. 05). Conclusion Bile reinfusion by nasojejunal canal in patients after bile external drainage surgery may improve postoperative inflammatory reaction and recovery of enterocinesia function.
