摘要
目的探讨自拟苦柏油和促愈油纱条外用对肛周脓肿术后创面愈合质量及肛门功能的影响。方法将本院100例行一次性根治术的肛周脓肿患者随机分为试验组与对照组。试验组给予自拟苦柏油和促愈油纱条外用,术后1~14 d给予自拟苦柏油纱条置于创面,从第15 d起至创面愈合期间给予促愈油纱条换药,换药1次/d。对照组术后1~14 d给予庆大盐水纱条换药,第15 d起至创面愈合期间采用紫草油纱条换药,换药1次/d。比较两组临床疗效、创面愈合质量、肛门功能及不良反应。结果试验组总有效率100.0%,显著高于对照组86.0%(P<0.05)。两组术后20 d创面渗液积分、创面水肿积分及创面疼痛积分均较术后2d显著降低,且试验组低于对照组(P<0.05)。两组术前、伤口愈合后肛管静息压、肛管最大收缩压比较差异均无统计学意义(P>0.05)。两组均未发生明显不良反应。结论自拟苦柏油和促愈油纱条外用可促进肛周脓肿术后创面愈合,且不影响术后肛门功能,安全性高,临床应用价值高。
Objective To investigate the effect of self-made asphalt combined with gauze on the wound healing quality and anal function after perianal abscess operation. Methods 100 patients with perianal abscess underwent one-time radical correction were randomly divided into experimental group and control group, with 30 patients in each group. The experimental group was treated with self-made asphalt and healing-promotion oil gauze for external use. From 1 to 14 d after the operation, self-made hard asphalt strips were placed on the wound surface. From 15 d to the wound healing, patients were treated with oil gauze（1 time/d）. The control group was given large saline gauze dressing from 1 to 14 d after operation. From the beginning of 15 d to the wound healing, they were treated with shikonin oil stripe（1 time/d）. The clinical efficacy, wound healing quality, anal function and adverse reactions were compared between the two groups. Results The total effective rate of the treatment group was 100.0%, which was significantly higher than that of the control group 86.0%（P〈 0.05）. 20 d after operation, the wound integral, wound edema score and wound pain score of the two groups were lower than those of 2 d postoperative, and these indices were lower in the treatment group than in the control group（P〈 0.05）. There was no significant difference in anal rest pressure and anal maximum systolic pressure between the two groups before operation and after wound healing（P〉 0.05）. There were no significant adverse reactions in the two groups. Conclusion Self-made tar combined with gauze can promote the healing of perianal abscess after operation, and the curative effect is significant. It does not influence the anal function after operation, and has the advantages of high safety.
