摘要
目的探讨冠脉血流重建前后的ST-T改变。方法研究对象共128例,均是本院接收的冠心病患者,研究采用回顾性分析法,收集整理并统计研究对象的临床资料,观察患者术前和术后的心电图ST-T变化。结果本次研究选取的128例接受CABG或PTCA术的患者冠脉血流重建前ST-T均异常,其中ST-T持续改变的患者75例,术后6例恢复正常,其余69例ST-T仍异常;ST段易变性异常的患者24例,术后4例恢复正常,其余20例ST-T仍异常;另29例患者术前存在心绞痛发作或运动试验诱发缺血性ST-T改变,术后28例恢复正常,1例术后仍异常。28例恢复正常的患者中,4例术后1年内复发缺血性ST-T改变,经临床证实这4例患者存在支架再狭窄。结论对于ST-T异常的患者来说,只有心绞痛和运动试验所引起的,接受冠脉血流重建后才能使患者的ST-T恢复正常。由此证明,对于易变性和持续性ST-T改变不宜诊断为心肌缺血,更不能作为接受冠脉血流重建的诊断依据。
Objective To investigate the changes of ST-T before and after coronary blood flow reconstruction. Methods There were 128 cases of coronary heart disease patients received in the hospital. The main research methods by retrospective analysis, which was collecting and organizing clinical data of all the subjects, to observe the changes of ST-T presented on ECG before and after surgery treatment. Results In this study, 128 patients who underwent CABG or PTCA were round to have abnormol ST-T, before coronary blood flow revascularization, among them, 75 patients with continuous change of ST- T, 6 cases of returned to normal after surgery, and the remaining 69 cases of ST-T was still abnormal. There were 24 patients with ST segment abnormal variability, and after surgery, 4 patients returned to normal, while the remaining 20 patients＇ ST-T was still abnormal. Another 29 patients was a change of the ischemic ST-T induced by angina or motor test before surgery, and 28 cases returned to normal, and 1 case was still abnormal after surgery. Of the 28 patients who returned to normal, 4 of them had recurrent ischemic STT changes within 1 year after surgery, and the 4 patients were clinically confirmed to had stent restenosis. Conclusion For patients with abnormal changes of ST-T, only the angina and the exercise test can cause ST-T changes of patients recover to normal after the coronary blood flow reconstruction. Therefore, it is proved that it is not suitable for the diagnosis of myocardial ischemia, and the diagnosis of coronary blood flow reconstrucyion.
