老年2型糖尿病患者血红蛋白及红细胞水平与骨质疏松症的关系 被引量:6

The relationship between hemoglobin,red blood cells and osteoporosis in elderly patients with type 2 diabetes mellitus

摘要 目的探讨老年2型糖尿病患者红细胞、血红蛋白水平与骨质疏松症的关系。方法选取218例老年2型糖尿病患者,根据骨密度检查将患者分为骨量正常组(49例)、骨量减少组(114例)、骨质疏松组(55例)。采用血常规检查检测全血细胞计数(CBC),采用双能X线骨密度测量腰椎、股骨颈、Ward三角、大粗隆、全髋骨密度,分析骨质疏松症组血红蛋白(Hb)、红细胞水平(RBC)与骨质疏松症的关系。结果 3组性别、吸烟史、饮酒史差异有统计学意义(P<0.05);骨质疏松组RBC、Hb低于骨质减低组和骨量正常组,差异有统计学意义(P<0.05)。骨质疏松组L_1~L_4、股骨颈、Ward三角、大粗隆、全髋骨密度值低于骨量减少组、骨量正常组,差异有统计学意义(P<0.05)。股骨颈、Ward三角、大粗隆、全髋骨密度(BMD)与Hb、RBC呈正相关(r=0.279、0.217、0.208、0.185,P<0.05),股骨颈、Ward三角、全髋BDM与年龄呈负相关(r=-0.235、-0.189、-0.145,P<0.05)。结论老年2型糖尿病骨质疏松症患者Hb水平降低,与骨密度相关。低水平的Hb可能是老年2型糖尿病骨质疏松的重要危险因素。 Objective To investigate the relationship between red blood cells, hemoglobin and osteoporosis in elderly patients with type 2 diabetes mellitus(T2DM). Methods Two hundred and eighteen elderly patients with T2DM were collected in this study, and divided into the normal bone mass group (49 cases), osteopenia group (114 cases) and osteoporosis group (55 ca- ses) according to bone mineral density examination. The relationship between hemoglobin, red blood cell levels and osteoporosis was analyzed. Results Sex, smoking history and drinking history were significantly different among the three groups (P 〈 0. 05 ). The red blood ceils, hemoglobin in the osteoporosis group were lower than those of the osteopenia group and the normal bone mass group, and the differences were statistically significant(P 〈0. 05). The BMD of L1 -L4, femoral neck, Ward triangle, greater trochanter and total hip in the osteoporosis group were lower than those of osteopenia group and the normal bone mass group, and the difference was statistically significant( P 〈 0. 05 ). There was a positively correlation between the BMD of femoral neck, Ward triangle, greater trochanter and total hip and hemoglobin, red blood cells( r = 0. 279, 0. 217, 0. 208, 0. 185, P 〈 0. 05 ). There was a negatively correlation between the BMD of femoral neck, Ward triangle, hip bone mineral density and age ( r = - 0. 235, - 0. 189, - 0. 145, P 〈 0. 05 ). Conclusion Hemoglobin is decreased in the elderly T2DM patients with osteoporosis and correlated with BMD. Low level of hemoglobin may be an important risk factor for osteoporosis in elderly patients with T2DM.
