600例单胎新生儿和孕母血清25-羟维生素D水平检测情况及其影响因素分析 被引量:5

Detection of serum 25-hydroxy vitamin D levels in 600 single live birth neonates and their mothers and analysis on the influencing factors

摘要 目的分析600例单胎新生儿及其孕母血清25-羟维生素D水平检测情况及其影响因素。方法选取2015年5月-2016年4月在四川绵阳四○四医院儿保门诊就诊的600例新生儿为研究对象,分别采集每位新生儿及其孕母各5ml空腹静脉血,以全自动化学发光免疫分析仪检测25-羟维生素D水平,并分析其影响因素。结果 600例新生儿平均25-羟维生素D水平为(12.93±7.72)ng/ml,其中25-羟维生素D不足者314例(52.33%),充足者94例(15.67%),缺乏者192例(32.00%)。孕母平均25-羟维生素D水平为(26.78±9.82)ng/ml;其中25-羟维生素D不足者159例(26.50%),充足者53例(8.83%),缺乏者388例(64.67%)。单因素分析显示:新生儿25-羟维生素D水平与新生儿胎龄、出生体质量、日龄、是否有黄疸、孕母孕期日晒时间、孕中晚期补充维生素D剂量、孕母产前BMI、孕母年龄等因素相关,差异有统计学意义(P<0.05)。多因素分析显示:新生儿出生体质量、日龄、孕母年龄、孕母25-羟维生素D水平、孕中晚期补充维生素D剂量是新生儿25-羟维生素D水平的影响因素(OR>1)。结论单胎新生儿血清25-羟维生素D水平普遍偏低,新生儿血清25-羟维生素D水平与其出生体质量、日龄、孕母年龄、孕母25-羟维生素D水平、孕中晚期补充维生素D剂量呈正相关关系。 Objective To analyze the detection of serum 25-hydroxy vitamin D levels in 600 single live birth neonates and their mothers and the influencing factors. Methods A total of 600 neonates were selected from Child Health Outpatient of Sichuan Mianyang 404 Hospital from May 2015 to April 2016,then 5 ml fasting venous blood samples were obtained from these neonates and their mothers. Fully automatic chemiluminescence immune detection system was used to detect the levels of serum 25-hydroxy vitamin D,and the influencing factors were analyzed. Results The average level of 25-hydroxy vitamin D in 600 neonates was( 12. 93 ± 7. 72) ng/ml. Among 600 neonates,there were 314 neonates( 52. 33%) with insufficient 25-hydroxy vitamin D,94 neonates( 15. 67%) with sufficient 25-hydroxy vitamin D,and 192 neonates( 32. 00%) with lack of 25-hydroxy vitamin D. The average level of 25-hydroxy vitamin D in mothers of these neonates was( 26. 78±9. 82) ng/ml. Among these mothers,there were 159 mothers( 26. 50%) with insufficient 25-hydroxy vitamin D,53 mothers( 8. 83%) with sufficient 25-hydroxy vitamin D,and 388 mothers( 64. 67%) with lack of 25-hydroxy vitamin D. Univariate analysis showed that the average level of serum 25-hydroxy vitamin D in neonates was significantly correlated with fetal age,neonatal birth weight,day age,complicated with jaundice or not,gestational solarization time,dose of vitamin D supplementation during the second and the third trimesters of pregnancy,progestational body mass index,and maternal age( P〈0. 05). Multivariate analysis showed that neonatal birth weight,day age,maternal age,maternal 25-hydroxy vitamin D level,dose of vitamin D supplementation during the second and the third trimesters of pregnancy were influencing factors of average level of serum 25-hydroxy vitamin D in neonates( OR1). Conclusion The levels of serum 25-hydroxy vitamin D in single live birth neonates are generally low,which are positively correlated with neonatal birth weight,day age,maternal age,maternal 25-hydroxy vitamin D level,dose of vitamin D supplementation during the second and the third trimesters of pregnancy.
