摘要
目的了解新疆维吾尔自治区0-6岁儿童甲型肝炎（甲肝）疫苗（Hepatitis Avaccine，HepA）接种率和甲肝抗体水平。方法采用多阶段抽样方法从全区94个县抽取0-6岁儿童进行HepA接种调查，并采集血清标本测定甲肝病毒（HAV）IgG抗体，描述性分析HepA接种率和HAV抗体水平的分布特征。结果适龄儿童HepA调查接种率为75．6％（3255／3624），不同地区、年龄、民族的接种率分别为71．0％-97．5％（X^2=171．74，P〈0．001）、3．1％-94．8％（X^2=2586．80，P〈0．001）、88．9％-96．1％（X^2=4．25，P=0．51）。儿童HAV抗体阳性率为82．9％（3526／4252），抗体几何平均浓度为296．7mIU／ml，不同地区、年龄、民族的抗体阳性率分别为71．5％-91．4％（X^2=99．91，P〈0．001）、59．5％-92．O％（X^2=502．54，P〈0．001）、70．0％-85．2％（x。=52．30，P〈0．001）。结论新疆0-6岁儿童HepA接种率和HAV抗体水平存在地区、年龄差异。需实施综合防控策略，加强儿童HepA常规免疫和重点地区或人群的查漏补种工作。
Objective To determine coverage of hepatitis A vaccine （HepA） and antibody levels against hepatitis A among children under 6 years of age in Xinjiang. Methods We used a multi-stage sampling strategy to select 0-6 year olds from 94 counties for a HepA coverage survey and to collect serum samples for measuring IgG antibody against hepatitis A virus （HAV）. We used descriptive analyses for HepA coverage and HAV antibody levels. Results Overall coverage of HepA in subjects was 75.6% （3 255/ 3624）, ranging from 71.0%-97.5% （x^2 =171.74, P 〈0.001） by region, 3.1% -94. 8% （x^2 = 2 586. 80, P 〈0. 001 ） by age, and 88.9%-96. 1% （x^2 =-4.25, P =0. 51 ） by race/ethnicity. The pos- itivity rate of HAV antibody in subjects was 82.9% （3 526/4 252） , and the geometric mean concentra- tion of antibody was 296.7 mIU/ml. Positivity rates ranged from 71.5% -91.4% （ x^2 = 99.91, P 〈 0.001） by region, 59.5% -92.0% （x^2 =502.54, P 〈0.001） by age, and 70. 0% -85.2% （x^2 = 52.30, P 〈 0. 001 ） by race/ethnicity. Conclusions HepA coverage and HAV antibody levels among children aged 0-6 years in Xinjiang varied with region and age. We should implement integrated preven- tion and control strategies to strengthen routine HepA immunization for children and conduct supplementa- IT immunization activities for priority areas and populations.
