摘要
目的观察加味大柴胡汤治疗急性胆囊炎(胆腑郁热证)的临床疗效,并探讨其作用机制。方法将患者86例按随机数字表法分为观察组与对照组各43例,对照组给予常规西医治疗,观察组在常规西医治疗的基础上给予加味大柴胡汤治疗,所有患者均治疗4周。比较两组的临床疗效、中医证候缓解时间及C反应蛋白(CRP)、白细胞计数(WBC)、中性粒细胞比例(NEUT%)。结果治疗4周后,观察组总有效率为93.02%,明显高于对照组的79.07%(P<0.05);中医证候(腹痛腹胀、纳差、恶心呕吐、发热)缓解时间明显低于对照组(P<0.05);治疗后两组的CRP、WBC、NEUT%均较治疗前降低,且观察组改善程度明显优于对照组(P<0.05)。结论加用加味大柴胡汤可有效降低急性胆囊炎(胆腑郁热证)患者CRP、WBC、NEUT%,改善腹痛、纳差等中医证候,提高临床疗效。
Objective： To observe the clinical effect of modified Dachaihu Decoction on acute cholecystitis with syndrome of heat stagnation in gallbladder. Methods： Eighty-six patients with acute cholecystitis with syndrome of heat stagnation in gallbladder were randomly divided into the observation group（n = 43） and the control group（n = 43）. The patients of the control group were treated by western medicine,while the observation group were treated by modified Dachaihu Decoction on the basis of Western medicine for 4 weeks. The clinical efficacy,relief time of TCM symptom,C-reactive protein,white blood cell count and neutrophil ratio were compared between the two groups. Results： After 4 weeks＇ treatment,the clinical curative effect in the observation group was significantly higher than that of the control group（P〈0.05）,while the relief time of TCM symptoms（abdominal distension,anorexia,nausea and vomiting,fever） was significantly lower（P〈0.05）. The levels of C-reactive protein,white blood cell count and neutrophil ratio decreased（P〈0.05）,and the observation group improved significantly better than the control group（P〈0.05）. Conclusion： Modified Dachaihu Decoction can reduce the C-reactive protein,white blood cell count and neutrophil ratio in patients with acute cholecystitis and improve the clinical symptoms.
