口服1200 IU维生素D_3对维生素D缺乏老年人血清维生素D、骨密度及下肢功能的影响 被引量:2

摘要 目的探讨口服1200 IU维生素D_3对维生素D缺乏老年人血清25羟基维生素D[25(OH)D]水平、骨密度及下肢功能的影响。方法选取2015年10月~2016年12月我院收治的老年人维生素D缺乏的病例56例为研究对象。按照研究者是否服用维生素D_3分成对照组(n=29)和观察组(n=27),对照组研究者每天晒太阳30 min并适当运动,观察组研究者在对照组基础上口服维生素D_3和钙尔奇D。比较两组干预前及干预后6、12个月的25(OH)D、骨钙素、甲状旁腺激素水平,骨密度、下肢步行时间。结果两组患者干预后6、12个月的血钙、血清25(OH)D、骨钙素、骨密度高于干预前,且观察组干预后6、12个月的上述指标均高于对照组(P<0.05)。两组患者干预后6、12个月的PTH水平低于干预前,且观察组干预后6、12个月的PTH水平低于对照组(P<0.05)。两组干预后的下肢肌力均较干预前得到良好改善(P<0.05),观察组干预后6、12个月的8步行走时间长于对照组,差异有统计学意义(P<0.05)。结论口服1200 IU维生素D_3有助于骨健康、改善骨密度、提高下肢功能,具有一定的临床应用价值,值得推广应用。 Objective To investigate the influence of oral administration of 1200 IU Vitamin D3 on serum 25-hydrox- yvitamin D [25(OH)D],bone mineral density (BMD) and lower extremity function in elderly patients with vitamin D-deficient.Methods From October 2015 to December 2016,56 elderly patients with vitamin D-deficient treated in our hos- pital were selected as research subjects.According to vitamin D3 took or not,they were divided into the control group (n= 29) and observation group (n=27).In the control group,30 min sun bathe daily and proper exercise were necessary.In the observation group,on the basis of the control group,oral administration of Vitamin D3 and Cahrate D was added.25(OH) D,osteocalcin,parathyroid hormone,BMD,and walking time of lower extremities were compared between the two groups before intervention,and after 6 and 12 months intervention.Results The serum calcium,serum 25 (OH)D,osteocalcin and BMD of the two groups were higher than those before intervention,and after 6 and 12 months intervention,and the above-mentioned indexes in the observation group were all higher than those in the control group (P〈0.05).PTH level after 6 and 12 months intervention in the two groups was lower than that before intervention,and PTH level of in the observation group after 6 and 12 months intervention was lower than in the control group (P〈0.05).The muscle strength of the two groups after intervention was better than that before intervention (P〈0.05).The 8 step walking time after 6 and 12 months intervention in the observation group Was longer than that in the control group (P〈0.05).Conclusion O- ral administration of 1200 IU vitamin D3 is beneficial to bone health,and improve BMD as well as the lower limbs function,which has a certain clinical application value and is worthy of promotion and application.
