摘要
目的探讨青少年肥胖与糖代谢异常发生的关系。方法将2016年6月-2017年3月本院收治的486例行健康体检的青少年作为研究对象,测量其身高、体质量,根据不同体质量指数(BMI)将青少年分为体质量过低、正常体质量、超重、肥胖。比较不同体质量指数人群血糖水平、胰岛素功能的差异,并分析肥胖糖代谢异常相关影响因素。结果肥胖、超重青少年服糖后0.5 h、1 h、2 h、3 h血糖水平高于正常体质量、体质量过低者;肥胖、超重青少年胰岛素水平、HOMA-IR水平高于正常体质量、体质量过低者,其HBCI水平低于正常体质量、体质量过低者,差异均有统计学意义(P<0.05)。TC、胰岛素抵抗指数、HDL、腰臀比、BMI是肥胖青少年糖代谢异常独立影响因素(P<0.05)。结论青少年肥胖存在一定程度的糖耐量异常、胰岛素功能障碍,临床应针对肥胖糖代谢异常的危险因素进行针对性干预措施。
Objective To explore the relationship between adolescent obesity and abnormal glucose metabolism. Methods From April 2016 to March 2017,486 healthy juveniles admitted to our hospital were selected as subjects. We measured the height and weight,and divided the adolescents from underweight,normal weight,overweight and obesity according to different body mass index. To compare the differences of blood glucose level,insulin function and obesity cognition among different body mass index groups,and to analyze the influencing factors of abnormal glucose metabolism. Results The blood glucose levels of blood sugar at 0. 5 h,1 h,2 h and 3 h in obesity and overweight adolescents were higher than normal weight and underweight ones. The levels of insulin,HOMA-IR in obesity,overweight adolescents were higher than normal body weight,underweight ones,and the HBCI level was lower than that of normal body weight and underweight adolescents,with the differences above all statistically significant( P〈0. 05). TC,insulin resistance index,HDL,waist-to-hip ratio,BMI were independent influencing factors for abnormal glucose metabolism in obese adolescents. Conclusion Adolescent obesity has a certain degree of impaired glucose tolerance,insulin dysfunction,so the clinical should be targeted at interventions on the risk factors for obesity and abnormal glucose metabolism,clinical intervention should be targeted for obesity and abnormal glucose metabolism in risk factors.
