膈肌松解技术联合吸气肌训练对脑卒中患者肺功能的影响 被引量:14

摘要 目的:研究膈肌松解技术联合吸气肌训练对脑卒中患者肺功能的影响。方法:将60例脑卒中患者按照随机数字表法分为膈肌松解技术联合吸气肌训练组(简称联合训练组)、吸气肌训练组和对照组各20例。3组均进行常规的康复训练(OT、PT),对照组只进行常规康复,吸气肌训练组在常规康复的基础上进行吸气肌训练,联合训练组在吸气肌训练的基础上进行膈肌松解,3组在训练前后均进行肺功能检测,包括用力肺活量(FVC)、第一秒用力呼气量(FEV1)、最大通气量(MVV)、最大吸气压(MIP)、吸气峰值流速(PIF)。结果:经过12周的训练,联合训练组和吸气肌训练组均能显著提高患者的FVC、FEV1、MVV、MIP、PIF(P均<0.05),与对照组比较差异具有统计学意义(P<0.05);联合训练组在改善患者的FEV1、MVV、MIP方面优于吸气肌训练组(P<0.05)。结论:与单独的吸气肌训练相比,膈肌松解技术联合吸气肌训练能够显著提高脑卒中患者的肺通气功能和吸气肌的功能,可作为有效的干预手段。 Objective: To study the effect of diaphragm release technique combined with inspiratory muscle training on pul-monary function of stroke patients. Methods: A total of 60 stroke patients were randomly divided into diaphragm release technique combined with inspiratory muscle training group (combined training group), inspiratory muscle training group and control group, 20 cases in each group. All of three groups were given regular rehabilitation training (OT, PT), the combined training group treated by in- spiratory muscle training on the basis of diaphragm release technique, the inspiratory muscle training group treated on the basis of regu- lar rehabilitation for inspiratory muscle training, the control group only treated by routine rehabilitation. Pulmonary function was tested before and after treatment, including forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), maximal voluntary ventilation (MVV), maximal inspiratory pressure (MIP), peak of inspiratory flow (PIF). Results: After 12 weeks of training, FVC, FEV1, MVV, MIP, PIF both in the combined training group and the inspiratory muscle training group were significantly improved compared with those in the control group (P〈0.05). FEV1, MVV and MIP in the combined training group were significantly improved than those in the inspiratory muscle training group (P〈0.05). Conclusion: The diaphragm release technique combined with inspiratory muscle training can significantly improve the patients' pulmonary ventilation function and inspiratory muscle function, which can be used as effective interventions in clinical.
