摘要
目的探讨甘精胰岛素结合阿卡波糖治疗对老年糖尿病患者糖化血红蛋白和空腹C肽的影响。方法 90例老年糖尿病患者随机分为对照组(45例)给予常规阿卡波糖治疗,实验组(45例)给予甘精胰岛素结合阿卡波糖治疗。比较两组血糖、糖化血红蛋白、空腹C肽水平及治疗有效率,检测治疗安全性并进行比较。结果与治疗前比较,两组空腹血糖、餐后2 h血糖水平均明显下降,且治疗后实验组各项指标水平均明显低于对照组(均P<0.05);实验组平均血糖达标时间显著短于对照组(P<0.05);与治疗前比较,两组糖化血红蛋白水平明显下降,空腹C肽值明显上升,且治疗后实验组各项指标水平均明显优于对照组(均P<0.05);实验组总有效率93.33%(42/45)显著高于对照组[82.22%(37/45),P<0.05];实验组不良反应发生率均明显低于对照组(P<0.05);两组治疗前后肝肾功能各项指标均未出现明显变化,且不同时间组间比较差异无统计学意义(均P>0.05)。结论甘精胰岛素结合阿卡波糖明显改善老年糖尿病患者糖化血红蛋白和空腹C肽水平,提高治疗有效率且不良反应轻微。
Objective To investigate the clinical effect of insulin glargine combined with acarbose in the treatment of senile diabetes. Methods The clinical data of 90 cases of elderly patients with diabetes mellitus were collected and analyzed retrospectively. The data collection time was from December 2015 to December 2016. Patients were randomly divided into two groups according to the time of admission, the reference group (45 cases) received routine treatment, the experimental group (45 cases) received insulin glargine combined with acarbose treatment. The blood glucose levels, glycosylated hemoglobin levels, fasting C peptide values and treatment efficiency were compared between the two groups. Results Compared with before treatment, two groups of fasting blood glucose, postprandial 2 h blood glucose levels were obviously decreased after treatment, indicators of experimental group were significantly lower than those of reference group ( P〈0. 05 ). The mean time of blood glucose in experimental group was significantly shorter than that of reference group (P〈0.05). Compared with before treatment, HbAlc levels of two groups were significantly decreased, fasting C peptide detection levels were significantly increased, and after treatment indicators of observed level group were significantly better than those of reference group ( P〈0.05 ). According to the curative effect, the total effective rate was 93.33% of the experimental group, which was significantly higher than that of the reference group (82.22%, P〈0.05 ). In the two groups, there was a certain gastrointestinal reaction and hypoglycemia, but the incidence of experimental group was significantly lower than that of reference group (P〈0.05). Liver and kidney function indexes of the two groups before and after treatment were detected and all indexes did not change significantly before and after treatment, and there were no significant differences at different time between the two groups (P〉0.05). Condusions Effects of insulin glargine combined with acarbose in the treatment for elderly patients with diabetes is significant, especially on improving blood glucose levels, glycosylated hemoglobin levels and fasting C peptide value, which can effectively improve the efficiency of treatment, and is effective treatment with mild adverse reactions.
