摘要
目的:探讨骶尾部藏毛窦的螺旋CT表现和临床应用价值,提高对本病的诊断水平。方法:回顾性分析2015年1月至2017年5月12例经病理证实为骶尾部藏毛窦的术前CT资料(7例行螺旋CT平扫,5例行螺旋CT平扫和增强扫描),总结其CT影像诊断特点。结果:本组12例中男11例女1例,平均年龄23.1±4.4岁。12例骶尾部藏毛窦呈卵圆形包块影,均位于骶5-尾1椎体水平正中臀间裂皮下浅表脂肪内,大小约1.1cm×1.2cm至3.5cm×3.9cm,平扫10例病灶与肌肉呈等或稍低于密度,2例病灶密度稍高于肌肉密度,5例增强扫描病灶呈不均匀强化,边界欠清楚,其中3例可见索条状低密度窦道影,长约1.1cm-2.7cm,窦道开口于皮肤并朝向头侧,所有病变未累及肌肉组织并且与骶尾椎椎管和肛管无沟通。结论:螺旋CT可以准确判断骶尾部藏毛窦的位置、范围和深度及其与邻近组织结构的关系,有助于鉴别诊断和临床手术治疗,可作为基层医院术前常规检查。
Objective:To investigate sprial CT manifestations and clinical application value of sacrococcygeal pilonidal sinus and to increase the diagnostic level of the disease.Methods:The sprial CT data of 12 preoperative patients with sacrococcygeal pilonidal sinus of pathologically-confirmed were retrospectively reviewed from January 2015 to May 2017( spiral plain CT scan was performed in 7 patients,and spiral plain CT scan and enhanced CT scan was done in 5 ones).Results:The 12 patients( 11 males and 1 female) were with an average age of 23.1 ± 4.4 years.All of sacrococcygeal pilonidal sinus in 12 patients were round or oval masses and were located in the subcutaneous fat layer of intergluteal cleft between the 5 th sacral and 1 st coccygeal vertebrae.The size of sacrococcygeal pilonidal sinus were between 1.1 cm × 1.2 cm and 3.5 cm ×3.9 cm.10 sacrococcygeal pilonidal sinuses in spiral plain CT scan were equal density or slightly lower density than the muscle,and 2 ones were slightly higher density than the muscle.The spiral enhanced CT scan showed inhomogeneous enhanced lesions and obscure boundary in 5 sacrococcygeal pilonidal sinuses.The lower density sinus tracts with 1.1 cm - 2.7 cm were showed in 3 patients in the spiral enhanced CT scan,which opened to skin and toward the head.None of all sacrococcygeal pilonidal sinus involved the muscle tissue and interlinked with the spinal canal and anal canal.Conclusion:Spiral CT can not only accurately determine the location、the range and the depth of sacrococcygeal pilonidal sinus,but also evaluate the relationship of it and its surrounding tissue structure.Spiral CT can contribute to the differential diagnosis and the operation plan of making,it can be used as preoperative routine diagnostic methods in primary hospitals.
