术中超声辅助手术治疗皮层下小型海绵状血管瘤

摘要 目的探讨术中超声辅助显微手术治疗皮层下小型海绵状血管瘤的临床价值。方法回顾性分析山东省立第三医院2013-10—2017-01收治的12例皮层下小型海绵状血管瘤患者的临床资料,术中采用超声确定病变位置行开颅显微手术切除,对超声在显微神经外科手术中辅助定位病变的价值进行评价。结果 12例病变均定位准确并完全切除,并最大限度减少了对脑组织损伤,术后病理证实术前诊断均为海绵状血管瘤,术前症状均有不同程度改善。结论对于皮层下小型海绵状血管瘤的手术,术中超声辅助定位病变精确,操作简便,能够有效辅助选择最佳手术路径,减少脑组织损伤。 Objective To evaluate the clinical value of intraoperative ultrasonography for real-time guidance when perform- ing microneurosurgical resection of small subcortical cavernous hemangioma. Methods The clinical data of 12 patients with small subcortical cavernous hemangioma were analyzed retrospectively. During the surgery, we selected the best surgical approach and microsurgery resection of the lesions with ultrasonic assistance and evaluated the clinical value of auxiliary positioning lesions in the neurosurgery. Results 12 cases of microsurgery were performed with ultrasonic assistance and all the lesions were totally rescected and maximally reduced the brain tissue damage. Postoperative pathology confirmed that the preoperative diagnosis of cavernous hemangioma and preoperative symptoms were improved in varying degrees. Conclusion For small subcortical cavernous hemangioma operation,precise intraoperative ultrasound is of great value in positioning pathological changes, easy to operate, can effectively assist the surgeons to choose the best surgery approach and reduce brain tissue damage.
