摘要
目的探讨健脾止泻汤治疗小儿腹泻的疗效及其对血清炎症因子与免疫功能的影响。方法选取2015年1月—2016年7月期间儿科门诊部腹泻小儿116例,采用随机数字表法将其分为两组,对照组采用常规治疗,治疗组在对照组基础上联合应用健脾止泻汤治疗,采用双抗体夹心酶联免疫吸附测定血清炎症因子白细胞介素(IL-10)、IL-6、肿瘤坏死因子α(TNF-α)水平,免疫荧光法测定CD4/CD8水平,免疫单扩散法测定免疫球蛋白A、G(IgA、IgG)水平。比较两组临床疗效、血清炎症因子与免疫指标的差异。结果治疗组治疗总有效率明显高于对照组,排便恢复正常时间、腹痛缓解时间、呕吐缓解时间与退热时间明显短于对照组,两组比较差异具有统计学意义(P<0.05)。治疗前,两组血清炎症因子与免疫指标比较差异无统计学意义(P>0.05),治疗后,两组患儿IL-10、IL-6、TNF-α均明显低于治疗前,CD4/CD8、IgA、IgG均明显高于治疗前(P<0.05),且治疗组治疗后均明显优于对照组(P<0.05)。结论健脾止泻汤治疗小儿腹泻的疗效显著,有助于显著降低炎症因子水平,提高免疫功能,值得临床推广应用。
Objective To explore the therapeutic effects of jianpi zhixie decoction on infantile diarrhea and the impacts on serum inflammatory factors and immune function. Methods From January 2015 to July 2016,116 cases of infantile diarrhea treated in the clinic of the Pediatric Department were collected and divided into the two groups according to the random number tables. In the control group,the routine treatment was adopted. In the observation group,on the base of the treatment as the control group,jianpi zhixie decoction was added. The double antibody sandwich enzyme-linked immunosorbent assay was used to determine the levels of IL-10,IL-6 and TNF-α. The immunofluorescence method was adopted to determine the level of CD4/CD8. The single radial immune diffusion was applied to determine the levels of IgA and IgG. The clinical therapeutic effects,the serum inflammatory factors and immune indexes were compared between the two groups. Results The total effective rate in the observation group was higher than the control group. The recovery time of normal defecation,the remission time of abdominal pain,the remission time of vomiting and the anti-febrile time in the observation group were shorter remarkably than the control group,indicating the significant differences（ P 0. 05）. Before treatment,the serum inflammatory factors and immune indexes were not different significantly between the two groups（ P 0. 05）. After treatment,the levels of IL-10,IL-6 and TNF-α were lower remarkably and the levels of CD4/CD8,IgA and IgG were higher remarkably as compared with those before treatment（ P 0. 05）. The results in the observation group were better remarkably than the control group（ P 0. 05）. Conclusion Jianpi zhixie decoction achieves the significant effects on infantile diarrhea and contributes to reducing the inflammatory factors and improving the immune function.Hence,this therapy deserves to be promoted in clinic.
