摘要
目的:探讨经尿道前列腺钬激光剜除术(HoLEP)后发生压力性尿失禁的危险因素。方法:回顾性分析2012年11月~2016年3月采用HoLEP治疗的1 180例良性前列腺增生(BPH)患者的临床资料。所有患者均由同一组手术医生完成。对可能引起术后压力性尿失禁的相关危险因素进行单因素分析及多因素Logistic回归分析。结果:HoLEP术后出现压力性尿失禁68例(5.76%)。单因素分析结果显示,患者年龄、是否合并糖尿病、前列腺体积及手术时间是影响术后发生压力性尿失禁的主要因素,而前列腺特异性抗原(PSA)、术前药物治疗、术前急性尿潴留及术后导尿管留置时间与术后是否发生压力性尿失禁无关。进一步多因素分析结果显示,年龄≥70岁、合并糖尿病及手术时间≥40min是术后发生压力性尿失禁的独立危险因素(P<0.05)。结论:与其他治疗BPH的手术操作比较,HoLEP在术后会引起一定比例的压力性尿失禁。高龄、合并糖尿病和手术时间增加是术后发生压力性尿失禁的独立危险因素。
Objective：To investigate the risk factors for postoperative stress urinary incontinence after holmium laser enucleation of the prostate（HoLEP）.Method：This is a retrospective study of 1180 patients undergoing HoLEP from November 2012 to March 2016 in our center.All the procedures were completed by the same group of surgeons.Univariate and multivariate logistic regression were used to investigate the risk factors associated with the occurrence of postoperative urinary incontinence.Result：Stress urinary incontinence after HoLEP was found in 68 patients（5.76%）.Univariate analysis presented that age,the presence of diabetes mellitus,prostate volume and operation time had impact on the occurrence of postoperative urinary incontinence,whereas prostate specific antigen（PSA）,preoperative medications,preoperative urinary retention and duration of catheter time after surgery were not significantly associated with postoperative urinary incontinence.On multivariate analysis,age over 70 years old,combined with diabetes mellitus and operation time over 40 min were statistically independent risk factors.Conclusion：Stress urinary incontinence occurred after HoLEP in some patients.Age,the presence of diabetes mellitus and operation time were associated with the occurrence of postoperative stress urinary incontinence.
