摘要
目的应用经会阴超声评估产后早期女性盆底功能,探讨不同分娩方式对女性盆底功能的影响,为能尽早的盆底功能康复提供依据。方法选取产后6~8周超声复查的初产妇98例作为观察组,其中经阴道分娩48例为顺产组,选择性剖宫产50例为剖宫产组;选取体检的30例未育女性作为对照组。分别观察3组女性静息状态及最大Valsaval动作时前腔室结构的改变,测量膀胱颈位置、膀胱尿道后角膀胱颈移动度、尿道旋转角,观察有无尿道内口漏斗形成。结果静息状态顺产组膀胱颈位置、膀胱尿道后角较对照组差异有统计学意义(P<0.05),剖宫产组较对照组差异无统计学意义(P>0.05)。最大Valsaval动作时膀胱颈移动度、尿道旋转角观察组均增大,其中顺产组较剖宫产组增大多,尿道漏斗形成率较高,差异有统计学意义(P<0.05)。结论经会阴超声可用于观察产后女性盆底结构及功能评估,为女性盆底功能障碍性疾病的早期诊断及治疗评估提供依据。
Objective To evaluate early pelvic floor function of postpartum women by transperineal ultrasonography,explore the effects of different delivery methods on female pelvic floor function, provide a basis for early rehabilitation of pelvic floor function.Methods Ninety-eight primiparous women receiving ultrasonic reeexamination during 6-8 weeks after delivery were selected as observation group,then they were divided into spontaneous labor group( 48 cases,transvaginal delivery) and cesarean section group( 50 cases,selective cesarean section); 30 nonparous women receiving physical examination were selected as control group. The structural changes of anterior chamber in the resting state and the maximal Valsaval movement in the three groups were observed. Bladder neck position,bladder urethral posterior angle,bladder neck mobility,and urethral rotation angle were measured. Internal urethral orifice funnel formation or not was observed. Results There were statistically significant differences in bladder neck position and bladder urethral posterior angle in the resting state between spontaneous labor group and control group( P〈0. 05),while there was no statistically significant difference between cesarean section group and control group( P〈0. 05). Bladder neck mobility and urethral rotation angle in the maximal Valsaval movement in observation group increased,especially in spontaneous labor group,the internal urethral orifice funnel formation was high,there were statistically significant differences( P〈0. 05). Conclusion Transperineal ultrasonography can be used to observe pelvic floor structure,evaluate the function,and provide a basis for early diagnosis and treatment evaluation of female pelvic floor dysfunction.
