摘要
目的探讨截石斜仰卧位、斜仰卧位和完全俯卧位对复杂性肾结石病人经皮肾镜碎石术治疗效果的影响。方法选取复杂性肾结石病人180例,均应用经皮肾镜碎石术治疗,随机分为3组,各60例。A组病人采取截石斜仰卧位,B组病人采取斜仰卧位,C组病人采取完全俯卧位。观察并比较3组病人的临床疗效及并发症的发生情况。结果 3组手术时间比较,A组短于B、C组,B组短于C组,差异有统计学意义(F=8.753,P<0.01)。3组术中出血量、住院时间比较,差异均无统计学意义(P>0.05)。A组病人术中体位舒适率高于B组、C组,差异有显著性(χ~2=17.252、18.634,P<0.01),B组、C组舒适率比较差异无统计学意义(P>0.05)。3组结石清除率、并发症发生率比较差异无统计学意义(P>0.05)。结论复杂性肾结石病人经皮肾镜碎石术采用截石斜仰卧位、斜仰卧位和完全俯卧位的效果相当,但截石斜仰卧位较其他两种体位能显著缩短手术时间,提高病人体位舒适度。
Objective To investigate the impact of lithotomy position combined with oblique supine position,oblique supine position,and complete prone position on percutaneous nephrolithotomy in patients with complex nephrolithiasis.Methods A total of 180 patients with complex nephrolithiasis undergoing percutaneous nephrolithotripsy were randomly and equally divided into three groups：group A（lithotomy position combined with oblique supine position）,group B（oblique supine position）,and group C（complete prone position）.The clinical outcome and complications of three groups were observed and compared.Results Group A had a significantly shorter time of operation than groups B and C,and the time of operation in group B was significantly shorter than that in group C（F=8.753,P〈0.01）.There were no significant differences in intraoperative blood loss and length of hospital stay between three groups（P〈0.05）.The comfortable rate of intraoperative body position in group A was significantly higher than that in groups B and C（χ^2=17.252,18.634;P〈0.01）,but there was no significant difference in the comfortable rate between groups B and C（P〉0.05）.Moreover,there were no significant differences in stone clearance rate and complication incidence between the three groups（P〉0.05）.Conclusion Lithotomy position combined with oblique supine position,oblique supine position,and complete prone position have similar impact on percutaneous nephrolithotripsy in patients with complex nephrolithiasis.However,lithotomy position combined with oblique supine position can shorten the time of operation and improve the intraoperative comfort of patients compared with the other two body positions.
