摘要
目的分析规范化护理管理措施对腺样体肥大儿童的疼痛及生活质量的影响。方法选择2016年1月至12月在我院进行治疗的腺样体肥大儿童120例为研究对象。所有入选儿童被随机分为观察组和对照组,观察组儿童给予规范化护理管理措施,对照组儿童给予常规护理管理措施。观察2组儿童的的睁眼时间、呼吸恢复时间、手术时间、拔管时间、疼痛视觉模拟评分(VAS)、儿童阻塞性睡眠呼吸暂停疾病特异性生活质量调查法(OSA-18)评分以及并发症发生情况。结果观察组患儿的睁眼时间、呼吸恢复时间、手术时间以及拔管时间(5.7±1.4)min、(5.2±1.6)min、(6.3±1.6)min、(35.1±8.8)min均短于对照组(9.7±1.3)min、(8.7±1.7)min、(10.4±2.1)min、(58.6±12.4)min,差异有统计学意义(P<0.05);观察组患儿术后第1天、第3天、第5天的术后VAS疼痛评分(2.0±0.5)分、(1.8±0.4)分、(1.5±0.5)分均低于对照组(4.9±1.3)分、(3.7±1.1)分、(2.9±1.2)分,差异有统计学意义(P<0.05);观察组患儿经护理干预后睡眠障碍、情绪不佳、身体症状、白天功能以及对监护人的影响评分(3.64±0.62)分、(2.16±0.33)分、(0.53±0.11)分、(1.34±0.49)分、(3.21±0.55)分,均低于对照组(8.71±2.38)分、(6.85±2.17)分、(2.75±1.37)分、(4.81±1.15)分、(7.63±1.38)分,差异有统计学意义(P<0.05);观察组患儿经护理干预后的并发症发生率(5%)低于对照组(22%),经检验可知差异有统计学意义(P<0.05)。结论对腺样体肥大儿童采取规范化护理管理措施有助于手术的顺利进行,减轻患者术后疼痛,提高生活质量,且可减少并发症的发生,建议广泛推广应用于临床。
Objective To investigate the effect of standardized nursing management on pain and quality of life in children with adenoid hypertrophy. Methods A total of 120 children with adenoid hypertrophy, who were treated in our hospital between January 2016 and December 2016, were included as the subjects in the study. All the includ-ed children were randomly divided into the study group and control group. The study group was given standardized nursing management, and the control group was given routine nursing management. The duration of opening eyes, re-covery time of breathing, duration of operation, extubation time, VAS pain score, OSA-18 score, and incidence of complications were determined in both groups. Results The duration of opening eyes, recovery time of breathing, duration of operation,and extubation time in the study group were all lower than those in the control group(5.7±1.4)min vs(9.7 ±1.3)min,(5.2 ±1.6)min vs(8.7 ±1.7)min,(6.3 ±1.6)min vs(10.4 ±2.1)min,(35.1 ±8.8)min vs(58.6 ±12.4)min,and the test showed that there were statistically significant differences(P〈0.05). At 1,3 and 5 d after the treatment, the VAS pain scores in the study group were all lower than those in the control group(2.0±0.5) vs(4.9±1.3),(1.8±0.4) vs(3.7±1.1),(1.5±0.5) vs(2.9±1.2), and the test showed that there were statistically significant differences(P〈0.05). After the nursing intervention, the scores of sleep disorder, poor mood, physical symptoms, daytime function and guardian′s effect in the study group were all lower than those in the control group(3.64±0.62) vs(8.71±2.38),(2.16±0.33) vs(6.85±2.17),(0.53±0.11) vs(2.75±1.37),(1.34±0.49) vs(4.81±1.15),(3.21±0.55) vs(7.63±1.38), and the test showed that there were statistically significant differences(P〈0.05). The incidence of complications in the study group after the nursing intervention was lower than that in the control group(5% vs 22%), and the test showed that there was statistically significant difference(P〈0.05). Conclusion The standardized nursing management favors the successful process of operation in children with adenoid hypertrophy. It can relieve the postoperative pain, improve the quality of life, and reduce the incidence of complications, which justifies widespread use in clinical settings.
