摘要
目的探讨经皮撬拨复位空心螺钉治疗SandersⅡ、Ⅲ型跟骨骨折的效果。方法选取2012年4月至2017年12月我院收治的SandersⅡ、Ⅲ型跟骨骨折患者125例,按照手术方式的不同分为两组。观察组64例患者采用经皮撬拨复位空心螺钉内固定治疗,对照组61例患者采用切开复位钢板内固定治疗,比较两组患者的并发症发生率及相关临床指标。结果观察组的手术时间、术中出血量和住院时间均显著少于对照组(P<0.05)。观察组的术后并发症发生率显著低于对照组(P<0.05)。两组患者的术后跟骨宽度、Bohler角与Gissane角无统计学差异(P>0.05)。结论经皮撬拨复位空心螺钉内固定治疗SandersⅡ、Ⅲ型跟骨骨折的效果显著,可有效减少对患者的损伤,在缩短住院时间和降低并发症发生率等方面具有显著优势,值得临床应用推广。
Objective To explore the effect of percutaneous poking reduction and cannulated screw for the treatment of Sanders Ⅱ and Ⅲ calcaneal fractures. Methods 125 cases of patients with Sanders Ⅱ and Ⅲ calcaneal fractures admitted to our hospital from April 2012 to December 2017 were selected and divided into two groups according to the surgical methods. 64 cases of the observation group were treated with percutaneous poking reduction and internal fixation with cannulated screw, while 61 cases of the control group were treated with open reduction and internal fixation with plate. The incidence of complications and related clinical indicators were compared between two groups. Results The operation time, intraoperative blood loss amount and hospitalization time of the observation group were significantly lower than those of the control group (P 〈0.05). The incidence of postoperative complications of the observation group was significantly lower than that of the control group (P 〈0.05). No statistical difference was found between the two groups in the postoperative calcaneus width, Bohler angle or Gissane angle (P 〉0.05). Conclusions Percutaneous poking reduction combined internal fixation with cannulated screw for the treatment of Sanders Ⅱ and Ⅲ calcaneal fractures has significant efficacy, which can effectively reduce the damage to patients, and has significant advantages in shortening the hospitalization time and reducing the incidence of complications. It is worthy of clinical application and promotion.
