摘要
目的研究并探讨不同疗程舌下含服标准化粉尘螨变应原疫苗治疗儿童支气管哮喘伴变应性鼻炎的有效性和安全性。方法入组对象为2012年1月至2014年1月期间完成4年特异性免疫治疗的160例儿童支气管哮喘伴变应性鼻炎患儿,对其临床资料进行回顾性研究分析。160例儿童支气管哮喘伴变应性鼻炎患儿均接受舌下含服标准化粉尘螨变应原疫苗治疗,分别于患儿治疗1年后、治疗2年后、治疗3年后、治疗4年后对其进行随访评估。比较患儿在不同疗程的鼻炎控制效果、哮喘控制效果、鼻炎症状评分、哮喘症状评分、肺功能指标。结果治疗2年后、治疗3年后、治疗4年后患儿的鼻炎控制总有效率、哮喘控制总有效率均高于治疗1年后(P<0.05)。治疗2年后、治疗3年后、治疗4年后患儿的鼻炎症状评分均低于治疗1年后(P<0.05),其哮喘症状评分均高于治疗1年后(P<0.05)。治疗2年后、治疗3年后、治疗4年后患儿的肺功能指标均高于治疗1年后(P<0.05),治疗3年后、治疗4年后的肺功能指标均高于治疗2年后(P<0.05)。结论舌下含服标准化粉尘螨变应原疫苗可对儿童支气管哮喘伴变应性鼻炎起到显著的治疗效果,尤以3年疗程效果最好,可有效控制其鼻炎和哮喘症状,改善其肺功能。
Objective To study and investigate the effectiveness and safety of different treatment of sublingual standardized dust mite allergen vaccine in the treatment of bronchial asthma in children with allergic rhinitis. Methods From January 2012 to January 2014,160 children with allergic rhinitis were treated with bronchial asthma and allergic rhinitis after 4 years of specific immunotherapy in our hospital. The clinical data of 160 children with bronchial asthma associated with allergic rhinitis were analyzed retrospectively. All of them were treated with sublingual sublingual standardized dust mite allergen vaccine,and after one year of treatment,2 years of treatment,3 years of treatment,4 years of treatment they were followed up to evaluate the control effect of rhinitis,asthma control effect,rhinitis symptom score,asthma symptom score and pulmonary function index in children with different courses of treatment. Results After 2 years of treatment,after 3 years of treatment,after 4 years of treatment,the total efficiency of asthma control was higher than the total effective rate after 1 year of treatment（ P〈0. 05）. After 2 years of treatment,after 3 years of treatment,after 4 years of treatment,rhinitis symptoms scores were less than those after 1 years after treatment（ P〈0. 05）,the asthma symptom scores were higher than those after 1 years of treatment（ P〈0. 05）. After 2 years of treatment,after 3 years of treatment,after 4 years of treatment,pulmonary function index were higher than that after 1 years of treatment（ P〈0. 05）. After 3 years treatment and 4 years of treatment,the indexes of lung function were higher than that after 2 years of treatment（ P〈0. 05）. Conclusion The sublingual standardized dust mite allergen vaccine on bronchial asthma in children with allergic rhinitis has significant therapeutic effect,especially in the course of3,which can control the symptoms of rhinitis and asthma effectively,improve the lung function.
