摘要
目的 探讨研究经内镜逆行胰胆管造影(endoscopic retrograde cholangiopancreatography,ERCP)、十二指肠乳头小切开联合乳头括约肌气囊扩张术治疗肝外胆管结石的临床效果。方法选择2016年1月至2017年1月在本院治疗的肝外胆管结石患者86例,随机分为观察组和对照组,每组43例。对照组采用ERCP、十二指肠乳头小切开治疗,观察组在对照组的基础上联合应用乳头括约肌气囊扩张术治疗。比较两组患者的临床疗效以及各项临床指标。结果观察组总有效率为90.69%,明显高于对照组的74.41%,差异有显著性(P<0.05);观察组的手术时间、术中出血量以及住院时间均明显低于对照组,差异有显著性(P<0.05)。结论 ERCP、十二指肠乳头小切开联合乳头括约肌气囊扩张术治疗肝外胆管结石疗效较好,安全性高,值得临床推广应用。
Objective To investigate the clinical effect of ERCP(endoscopic retrograde cholangiopancreatography), duodenal papilla incision combined with papillary balloon dilatation in the treatment of extrahepatic bile duct stones. Method Eighty-six cases were randomly selected in our hospital for treatment of extrahepatic bile duct stones were divided into observation group and control group, 43 cases in each group. Among them, the control group was treated with ERCP, duodenal papilla small incision surgery, observation group in the control group on the basis of the application of papillary balloon dilatation therapy. The clinical curative effect and clinical indexes of two groups were compared. Result The total effective rate of observation group was significantly higher than the control group; the amount of bleeding, operation time of the observation group during operation and hospitalization time were better than the control group the corresponding values, the differences between the two groups of data comparison was statistically significant(P〈0.05). Conclusion ERCP,sphincterotomy combined with papillary sphincter balloon dilatation for treatment of hepatic bile duct stones better curative effect, shorten operation time of patients, reduce the bleeding, high safety, worthy of clinical application.
