黏附分子CD56、CD11b在急性髓系白血病中的表达及意义 被引量:2

The expression and clinical significance of adhesion molecule CD56 and CD11b in acute myeloid leukemia

摘要 目的:通过对49例急性髓系白血病(Acute Myeloid Leukemia,AML)患者和20例正常对照者骨髓单个核细胞表面抗原CD56和CD11b表达水平的检测,探讨其在急性髓系白血病中的表达及临床意义。方法:回顾性分析49例初诊AML患者的临床资料,流式细胞术(FCM)检测AML患者和正常对照者骨髓单个核细胞表面抗原CD56和CD11b的表达情况,χ~2分析率的差异,Pearson相关性分析CD56与CD11b的关系,差异具有统计学意义(P<0.05);结果:(1)CD56抗原在正常对照组中不表达,而在AML组中的表达率为38.8%,两者差异具有统计学意义(P<0.05);CD11b抗原在正常对照组和AML组的表达差异无统计学意义(P>0.05);(2)CD56+AML组的完全缓解(CR)率低于CD56-AML组(42.1%vs83.3%)(P<0.05),差异具有统计学意义;CD11b+AML组的缓解率低于CD11b-AML组(33.3%vs75%)(P<0.05),差异具有统计学意义。(3)Pearson相关性分析示两者显著相关(P<0.05),差异具有统计学意义。(4)收集CD56+CD11b+患者5例,2个疗程后仅1例达完全缓解。结论:白血病细胞免疫表型的检测有助于对AML预后评估,CD56、CD11b阳性表达的AML提示缓解率低,预后不良。 Objective: To detect the expression level of CD56 and CD11 b in bone marrow mononuclear cells of patients with acute myeloid leukemia( AML); and to investigate its expression in acute myeloid leukemia and its clinical significance. Methods:The clinical data of 49 patients with AML was retrospectively analyzed. Flow cytometry( FCM) was used to detect the expression surface of antigen CD56 and CD11 b of bone marrow mononuclear cell,the difference of their χ~2,Pearson correlation analysis of the relationship between CD56 and CD11 b.(P〈0. 05) was statistically significant. Results:①CD56 antigen was not expressed in the normal control group,while the expression in the AML group was 38. 8%,and the difference was statistically significant( P〈0. 05);but the expression of CD11 b antigen in normal control group and AML group was not statistically significant(P〈0. 05);②Complete remission( CR) in CD56 + AML group was lower than that of group CD56-AML(42. 1% vs83. 3%)(P〈0. 05),and the difference was statistically significant. CR in CD11 b + AML group was lower than that of CD11 b-AML group(33. 3% vs75%)( P〈0. 05),and the difference was statistically significant.③Pearson correlation analysis showed significant correlation of them( P〈0. 05),and the difference was statistically significant.④CD56 + CD11 b + patients were collected in 5 cases,and only one patient had CR after 2 courses of treatment. The detection of leukocyte immunophenotypes is useful for the prognosis of AML,and the positive expression of CD56 and AML suggests that the remission rate is low and the prognosis is poor in CD11 b.
