摘要
目的探讨磁敏感加权成像(SWI)、磁共振(MRI)常规序列及扩散加权成像(DWI)在大面积脑梗死出血性转化(HT)中的诊断价值。方法回顾性分析2015年1月至2017年8月新疆兵团第六师医院医学影像科收治的29例大面积脑梗死患者的影像资料,全部采用1.5T超导MRI常规T1WI、T2WI、FLAIR、DWI序列和SWI序列扫描,观察分析各个序列影像特点并进行比较。结果SWI序列对于大面积脑梗死HT的检出率显著高于MRI常规序列及DWI序列,差异有统计学意义(P〈0.05),T1WI联合DWI序列对于大面积脑梗死HT的检出率高于MRI常规序列及DWI序列,差异有统计学意义(P〈0.05),各MRI常规序列阳性检查率之间比较,差异无统计学意义(P〉0.05)。结论SWI序列在诊断大面积脑梗死HT中的出血灶具有高度的特异性,但显示病灶全貌需结合DWI序列及MRI常规序列,T1WI联合DWI序列能够基本全面显示大面积脑梗死后HT,在诊断中具有较高的诊断价值。
Objective To investigate the weighted imaging ( DWI ) and susceptibility weighted diagnostic value of conventional sequence, diffusion imaging (SWI)in the hemorrhagic transformation of massive cerebral infarction. Methods A retrospective analysis was performed on 29 patients with massive cerebral infarction who were admitted from January 2015 to August 2017. The sequence of T1WI, T2WI, flair water, DWI and SWI used 1.5T superconductive magnetic resonance to analyze and compared the char- acteristics and each sequence. Results The detection rate of SWI sequence was significantly higher than that of conventional MRI sequence and DWI, and the difference was statistically significant (P 〈 0. 05 ). The detection rate of T1WI combined with DWI was significantly higher than that of conventional MRI sequence and DWI(P 〈0. 05). There was no statistically significant difference between the positive rate of conven- tional MRI(P 〉 0. 05 ). Conclusion SWI is highly specific in diagnosis of hemorrhagic transformation of massive cerebral infarction; it needs to be combined with DWI and conventional sequence of the overall pic- ture of the lesion. T1WI combined with DWI can basically display in diagnosis of hemorrhagic transforma- tion of massive cerebral infarction, and it has a high diagnostic value in the diagnosis as well.
