摘要
目的探讨葛根素联合神经节苷脂钠对突发性耳聋患者血液流变学和血管内皮功能的影响。方法选取医院2015年3月至2017年5月收治的突发性耳聋患者77例,采用随机数字表法分为对照组(38例)与研究组(39例)。对照组患者给予神经节苷脂钠、补充神经营养和补充维生素的常规治疗方式,研究组患者在对照组治疗基础上加用葛根素治疗,两组均治疗10 d。结果研究组治疗有效率为94.87%,显著高于对照组的78.95%(P<0.05)。治疗后,两组患者在250,500,1 000,2 000,3 000,4 000 Hz范围的听力阈值均较治疗前显著降低(P<0.05),且研究组显著低于对照组(P<0.05);两组患者的血浆黏度(PV)、红细胞聚集指数(EAI)、全血高切黏度(HS)、纤维蛋白原(FIB)、内皮素(ET)、可溶性血管细胞间黏附分子-1(sVCAM-1)含量值均显著低于治疗前(P<0.05),且研究组患者均显著低于对照组(P<0.05);两组患降钙素基因相关肽(CGRP)水平显著高于治疗前(P<0.05),且研究组显著高于对照组(P<0.05)。两组均无明显不良反应发生。结论葛根素联合神经节苷脂钠较单用神经节苷脂钠治疗突发性耳聋具有更好的临床疗效,可有效调节患者的血液流变学指标和血管内皮因子水平,促进听力恢复。
Objective To investigate the clinical effect of puerarin combined with ganglioside sodium on hemorrheology and vascular endothelial function in patients with sudden deafness. Methods Totally 77 patients with sudden deafness admitted to our hospital from March 2015 to May 2017 were selected and divided into the control group（ 38 cases） and the study group（ 39 cases） by the digital random grouping method. The control group was given the routine treatment of ganglioside sodium,supplementation of neuronutrition and vitamins,on this basis, the study group was treated with puerarin. The two groups were treated for 10 d. Results The effective rate of the study group was 94. 87%,which was significantly higher than 78. 95% of the control group（ P 〈 0. 05）. After treatment,the hearing thresholds in the two groups of patients at 250, 500, 1 000, 2 000, 3 000 and 4 000 Hz were significantly lower than those before treatment（ P 〈 0. 05）,and those in the study group were significantly lower than those in the control group（ P 〈 0. 05）. The contents of PV, EAI, HS,FIB,ET and sVCAM-1 in the two groups were significantly lower than those before treatment（ P 〈 0. 05）, and those in the study group were significantly lower than those in the control group（ P 〈 0. 05）. The levels of CGRP in the two groups were significantly higher than that before the treatment（ P 〈 0. 05）, and that in the study group was significantly higher than that of the control group（ P 〈 0. 05）. There was no obvious adverse reaction in the two groups. Conclusion Puerarin combined with ganglioside sodium has a better therapeutic effect than the simple application of ganglioside sodium in the treatment of sudden deafness, it can effectively regulate the hemorheological indexes and the levels of vascular endothelial factors,and promote the recovery of the patients’ hearing.
