摘要
[目的]探讨集束化护理对新生儿脐静脉置管并发症的干预效果。[方法]选取2016年6月—2017年5月我院新生儿科收治的152例行脐静脉置管术(UVC)的危重新生儿,2016年6月—2016年11月收治的74例患儿设为对照组,采用UVC常规护理。2016年12月—2017年5月收治的78例患儿设为观察组,采用依据UVC常见并发症的循证护理证据总结出的最佳集束化护理策略干预。比较两组患儿导管留置时间、并发症发生情况、体重增加情况等。[结果]观察组患儿导管留置时间明显少于对照组,差异有统计学意义(P<0.05)。对照组患儿20例发生并发症,发生率27.0%。观察组患儿12例发生并发症,发生率15.4%。观察组患儿并发症发生率低于对照组(P<0.05)。观察组患儿体重增加高于对照组(P<0.05)。[结论]集束化护理干预UVC,可以减少导管留置时间、降低并发症发生率且有助于患儿体重增加。
Objective To probe into the intervention effect of cluster nursing on complications of umbilical vein catheterization in neonates. Methods： A total of 152 critical neonates with umbilical vein catheterization （UVC） were selected in neonatal department of our hospital from June 2016 to May 2017.In them,74 neonates in control group received UVC routine nursing from June 2016 to November 2016 and 78 neonates in observation group used the best cluster nursing strategy by evidence based nursing evidences based on common complications of UVC from December 2016 to May 2017.The catheterization indwelling time,complications and weight gain were compared between the two groups. Results： The indwelling time of catheterization in observation group was significantly less than that in control group,the difference was statistically significant （ P 〈0.05）.There were 20 cases with complications in control group,and the incidence was 27%.12 cases with complications in observation group,and the incidence was 15.4%.The incidence of complications in observation group was lower than that in control group,and the difference was statistically significant （ P 〈0.05）.The weight gain of patients in observation group was higher than that in control group,the difference was statistically significant （ P 〈0.05）. Conclusions： Cluster nursing intervention of UVC could reduce catheterization indwelling time,reduce the incidence of complications,and contribute to the weight gain of neonates.
