经椎弓根椎体内植骨加短节段内固定治疗单节段骨质疏松性脊柱骨折疗效观察及术后近、中期随访研究 被引量:2

摘要 目的观察经椎弓根椎体内植骨加短节段内固定治疗单节段骨质疏松性脊柱骨折疗效及术后近、中期随访结果。方法选取2014年2月至2016年3月某院78例单节段骨质疏松性脊柱骨折患者,根据手术方案分组,各39例。对照组予以短节段内固定治疗,研究组予以经椎弓根椎体内植骨加短节段内固定。统计两组围手术期指标(术中出血量、手术时间、住院时间、骨折愈合时间)变化情况,于术后6个月、1年随访疼痛程度、椎体Cobb角变化情况、椎体高度丢失量。结果研究组术中出血量、手术时间、住院时间、骨折愈合时间与对照组相比,差异无统计学意义(P>0.05);术后6个月、1年研究组疼痛程度、椎体Cobb角、椎体高度丢失量均低于对照组,差异有统计学意义(P<0.05)。结论经椎弓根椎体内植骨加短节段内固定治疗单节段骨质疏松性脊柱骨折有利于缓解疼痛,减少椎体Cobb角及高度丢失,降低内固定物断裂或松动发生率。 Objective To observe the curative effect of single-segment osteoporotic vertebral fracture by the treat-ment of transpedical interbody bone grafting and short-segment fixation and the short-term and middle-term followupstudy of the operation.Methods 78 single-segment osteoporotic vertebral fracture patients admitted by a hospital fromFebruary 2014 to March 2016 were divided into the experimental and the control group in accordance with operationplan, with 39 cases in each group.The control group were given short-segment fixation while the experimental groupwere given transpedical interbody bone grafting and short-segment fixation.Perioperative indices of the groups (intraop-erative blood loss, operation time, hospitalization time and fracture healing time) were collected.6 months and 1 yearafter the operation, the degree of pain, vertebral Cobb angle, height loss of vertebral body were collected.Results In-traoperative blood loss, operation time, hospitalization time and fracture healing time of the experimental group showedno statistical significance from those of the control group( 〉0.05).6 months and1 year after the operation, the de-gree of pain, vertebral Cobb angle, height loss of vertebral body of the experimental group were lower than those ofthe control group and the difference showed statistical significance( 〈0.05).Conclusion Single-segment osteoporoticvertebral fracture by the treatment of transpedical interbody bone grafting and short-segment fixation eased the painsand decreased vertebral Cobb angle and height loss of vertebral body, lowering the occurance rate of broken or looseof internal fixation.
