摘要
目的分析深圳地区孕妇定植型B群链球菌(GBS)的血清型、毒力因子以及耐药基因的流行病学特征。方法收集2015年10月—2016年9月深圳市3所三级医院送检孕妇产前GBS筛查阳性菌株,采用K-B纸片扩散法进行药敏试验,多重PCR进行GBS血清学分型、α-样表面蛋白、菌毛蛋白及抗生素耐药基因检测。结果共分离GBS 56株,检出5个血清型(Ⅰa、Ⅰb、Ⅱ、Ⅲ、V),以Ⅲ型为主(60.7%)。不同血清型与特异表面蛋白及菌毛类型的分布存在相关性,如血清型Ⅰa与eps、Ⅰb与bca、Ⅲ与rib、V与alp2/3,血清型Ⅰb与PⅠ-2a、Ⅲ与PⅠ-2b及V与PⅠ-2a+PⅠ-1(均P<0.05)。56株GBS对青霉素全部敏感,对氯霉素、左氧氟沙星、红霉素、克林霉素、四环素耐药率分别为14.3%、23.2%、75.0%、67.9%及85.7%。结论深圳地区孕妇定植型GBS主要以Ⅲ型为主,不同的血清型与特定毒力基因有明显相关性。GBS对青霉素普遍敏感,对红霉素、克林霉素、四环素的耐药率较高。
Objective To analyze epidemiological characteristics of serotypes,virulence factors,and resistance genes of group B streptococcus( GBS) colonized in pregnant women in Shenzhen City.Methods Positive strains of prenatal GBS screening for pregnant women in three tertiary hospitals in Shenzhen City from October 2015 to September 2016 were collected,antimicrobial susceptibility testing was performed with Kirby-Bauer disk diffusion method,GBS serological typing,α-like surface protein,pili protein,and antimicrobial resistance genes were detected with multiplex polymerase chain reaction( multiplex PCR).Results A total of 56 GBS strains were isolated,5 serotypes were detected( Ⅰa,Ⅰb,Ⅱ,Ⅲ,V),which was dominated by type Ⅲ( 60.7%). There was correlation between different serotypes and specific surface protein as well as pili protein distribution,such as serotype Ia and eps,Ib and bca,Ⅲ and rib,V and alp2/3,serotype Ib and PI-2 a,Ⅲ and PI-2 b,as well as V and PI-2 a+PI-1( all P〈0.05). 56 strains of GBS were all sensitive to penicillin,resistance rates to chloramphenicol,levofloxacin,erythromycin,clindamycin,and tetracycline were 14. 3%,23. 2%,75. 0%,67.9%,and 85.7%,respectively.Conclusion The main type of GBS in pregnant women in Shenzhen City is type Ⅲ,there is a significant correlation between different serotypes and specific virulence genes. GBS is generally sensitive to penicillin,resistance rates to erythromycin,clindamycin,and tetracycline are high.
