摘要
目的探讨急性缺血性脑卒中患者溶栓后侧枝循环形成与预后。方法选择80例缺血性脑卒中患者,均进行溶栓治疗。统计溶栓治疗后侧枝循环形成情况,并评估患者预后情况。结果溶栓治疗后有侧支循环形成者45例(56.25%),无侧支循环者35例(43.75%)。有侧支循环形成者梗死病灶直径明显低于无侧支循环者(P<0.05),有侧支循环、无侧支循环者动脉狭窄程度分布比较差异有统计学意义(P<0.05)。与无侧支循环者比较,有侧支循环形成者溶栓治疗前、后NIHSS、mRS评分均显著较低,简易神经状态量表(MMSE)、蒙特利尔认知评估表(Mo CA)评分均显著较高(P<0.05)。有侧支循环者近期预后BI指数好于无侧支循环者(P<0.05)。结论急性缺血性脑卒中患者溶栓后侧枝循环形成对改善患者神经缺损、促进神经功能恢复及提高认知功能有重要意义,溶栓后是否有侧枝循环形成与患者预后密切相关。
Objective To analyze the relationship between formation of collateral circulation and prognosis of patients with acute ischemic stroke after thrombolysis.Methods A total of 80 patients with ischemic stroke were enrolled in the study. All patients were treated with thrombolysis.The formation of collateral circulation after thrombolysis was analyzed and the prognosis was evaluated.Results After thrombolysis,there were 45 cases（ 56. 25%） with collateral circulation formation and 35 cases（ 43. 75%） without.The diameter of infarction lesions in patients with collateral circulation was significantly smaller than those without（ P〈0. 05）,and there was significant differences in the degree of arterial stenosis between patients with collateral circulation and without（ P〈0. 05）. Compared with patients without collateral circulation,the NIHSS and mRS scores of patients with collateral circulation before and after thrombolysis were significantly lower while the scores of Mini Mental State Examination（ MMSE） and Montreal Cognitive Assessment（ Mo CA） were significantly higher（ P〈0. 05）.The prognostic BI index of the patients with collateral circulation was better than patients without（ P〈0. 05）.Conclusion The formation of collateral circulation is of great importance in relieving the neurological defects,promoting the recovery of neurological function and improving the cognitive function in patients with acute ischemic stroke after thrombolysis.The formation of collateral circulation is closely related to the prognosis of the patients after thrombolysis.
