胶质瘤部位与术前癫痫发生率相关性的Meta分析 被引量:3

摘要 目的系统评价胶质瘤部位与术前癫痫发生率的相关性。方法计算机系统检索PubMed、Embase、Web of Science、Cochrane Library等英文数据库,以及CBM和CNKI等中文数据库,全面收集关于脑胶质瘤的部位与术前癫痫发生率的相关性研究,检索时限截至2017年1月。采用Stata12.0软件进行Meta分析。结果共纳入15个研究,包括4 013例胶质瘤。Meta分析结果显示:额叶胶质瘤术前癫痫的风险较非额叶胶质瘤高(OR=1.43,95%CI 1.03~1.98,P=0.031),枕叶胶质瘤术前癫痫的风险较非枕叶胶质瘤低(OR=0.49,95%CI 0.26~0.93,P=0.014),而颞叶、顶叶、岛叶胶质瘤与术前癫痫无明显相关性(P>0.05)。结论胶质瘤部位与术前癫痫发生率具有一定相关性,额叶胶质瘤术前癫痫发生率高,枕叶胶质瘤最低,而颞叶、岛叶及顶叶胶质瘤术前癫痫风险暂未发现明显统计学差异。 Objective To evaluate the relationship between the glioma location and preoperative glioma-related epilepsy (GRE) inpatients with gliomas. Methods The databases including PubMed, EMBASE, Web of Science, China Biology Medicine, the CochraneLibrary and CNKI were systematically searched for original articles of relationship between the glioma location and preoperative GREpublished before the January, 2017 including randomized control study and case-control study. The meta-analysis was performed byStata 12.0 software. Results A total of 4, 013 participants in 15 population-based studies were included in this meta-analysis. Metaanalysis showed that gliomas in the frontal lobe were significantly associated with increased preoperative GRE risk (OR=1.43, 95% CI1.03~1.98, P=0.031) and the patients with gliomas in the occipital lobes had relatively low preoperative GRE (OR=0.49, 95% CI: 0.26~0.93, P=0.014). Conclusions Compared with those in the patients with temporal, parietal and insular lobes gliomas, the incidence ofpreoperative GRE was the highest in the patients with frontal gliomas, and the lowest in the patients with occipital gliomas. Our findingsprovide a clinical basis for the management of epileptic seizures in patients with gliomas. Due to the effect of confounding factors on theoutcomes of the present study, the above conclusions need to be further verified by large-scale high-quality research.
