摘要
目的探讨儿童嗜酸性膀胱炎的诊断、治疗方法,提高对该病的认识。方法回顾性分析2012年1月至2017年8月郑州大学第一附属医院收治的11例嗜酸性膀胱炎患儿的临床资料,并结合相关文献进行总结分析。结果 11例患儿均通过膀胱病理活检诊断为嗜酸性膀胱炎,其中经膀胱镜活检诊断5例,经超声/CT引导下膀胱全层活检诊断6例。所有患儿均接受糖皮质激素、抗组胺药物等保守治疗,症状及影像学表现明显减轻,其中1例患儿院外出现激素相关副作用,停用泼尼松片改单用抗组胺药,副作用逐渐消失,未见复发;随访1个月～5 a,3例复发(均有明确过敏原),复发率为27.3%,再次药物治疗及脱离过敏原治疗症状缓解。结论超声引导下膀胱全层活检可作为膀胱镜活检的重要补充/替代;糖皮质激素等药物治疗效果良好,不良反应发生率低;嗜酸性膀胱炎易复发,尤其是有明确过敏原的患儿,该病可能是一种变态反应疾病,随访和预防十分必要。
Objective To explore the diagnosis and treatment of eosinophilic cystitis in pediatric population and improve the understanding of this disease. Methods The clinical data of eleven children with eosinophilic cystitis admitted from January of 2012 to August of 2017 in the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed. Results Eleven cases were diagnosed as eosinophilic cystitis by biopsy of the bladder. Among them,5 cases were diagnosed by cystoscopic biopsy,and 6 cases were diagnosed by full-thickness biopsy guided by ultrasound/CT. All patients received glucocorticoids,antihistamines,and other conservative treatments. Significant relief in symptoms and imaging was observed. One patient had hormonal related side effects outside the hospital and he was discontinued from prednisone tablets with antihistamines alone. The side effects gradually disappeared,and no recurrence was found. In the follow-up period of 1 month to 5 years,3 cases relapsed（all had clear allergens）,and the recurrence rate was 27. 3%. The drug treatment and remission from the allergen treatment again relieved the symptoms. Conclusion Ultrasound-guided full-thickness bladder biopsy can be used as an important supplement/substitution for cystoscopic biopsy. Glucocorticoids and other drugs have good efficacy and low incidence of adverse reactions.Eosinophilic cystitis is easy to relapse,especially in children with clear allergens. The disease may be an allergic disease,and follow-up and prevention are necessary.
