摘要
目的探讨高频超声在甲状腺癌诊断和鉴别诊断中的应用价值。方法选取94例甲状腺癌患者作为观察组,并选取同期收治的90例甲状腺腺瘤患者作为对照组。术前对所有患者进行彩色多普勒超声诊断,比较2组患者的二维超声特点和彩色多普勒血流特点。结果在94例甲状腺癌患者中,经高频超声确诊甲状腺癌90例(95.74%),其中乳头状癌67例、滤泡状腺癌16例、髓样癌5例、未分化癌2例;误诊4例(4.26%),误诊为甲状腺瘤3例、结节性甲状腺肿1例。观察组患者的周边无回声、内部低回声、边界不清、微钙化灶、淋巴结转移率均显著高于对照组(P<0.05)。观察组患者的2~3级血流分布及血流参数(PSV、RI)均显著高于对照组(P<0.05)。结论甲状腺癌的高频超声图像具有低回声、边界不清、微小钙化、高PSV和高RI的特点,对于甲状腺癌的诊断和鉴别具有重要临床意义。
Objective To investigate the application value of high frequency ultrasound in the diagnosis and differential diagnosis of thyroid carcinoma. Methods 94 patients with thyroid cancer were selected as the observation group,90 patients with thyroid adenoma were selected as the control group. All patients were diagnosed by color Doppler ultrasound before operation,the characteristics of two-dimensional ultrasound and color Doppler flow were compared between the 2 groups. Results In 94 patients with thyroid cancer,90 cases by high frequency ultrasound diagnosed to be thyroid cancer(95. 74%),including 67 cases of papillary carcinoma,16 cases of follicular carcinoma,5 cases of medullary carcinoma,2 cases of undifferentiated carcinoma;4 cases were misdiagnosed(4. 26%),3 cases were misdiagnosed as thyroid adenoma,1 cases of nodular goiter. The number of no echo,low echo,unclear boundary,micro calcification and lymph node metastasis rate in the observation group were significantly higher than those of the control group( P〈0. 05). The 2 ~ 3 blood flow distribution and blood flow parameters( PSV,RI) in the observation group were significantly higher than those of the control group( P〈0. 05). Conclusion High frequency ultrasound images of thyroid carcinoma have the characteristics of low echo,unclear boundary,microcalcification,high PSV and high RI,which have important clinical significance in the diagnosis and differential diagnosis of thyroid carcinoma.
