摘要
目的观察超声诊断胎儿脊柱裂的价值及鉴别诊断开放性脊柱裂和闭合性脊柱裂的实用效果。方法选取2015年1月—2017年6月到该院进行胎儿脊柱裂诊断的患儿84例进行临床研究,所有患儿均给予超声检查,将84例脊柱裂胎儿按照脊柱裂的性质分为开放性脊柱裂组和闭合性脊柱裂两组,观察两组胎儿合并ArnoldChiari Ⅱ型异常的比例,两组胎儿头双顶径(BPD)、头围(HC)、枕大池消失、脑室扩张、小脑长度的情况并进行比较。结果 84例脊柱裂胎儿中,开放性脊柱裂患儿57例,其中合并Arnold-Chiari Ⅱ型异常52例,闭合性脊柱裂胎儿27例,无合并Arnold-Chiari Ⅱ型异常。开放性脊柱裂胎儿BDP<-2SD、HC<-2SD、枕大池消失、双侧脑室扩张以及小脑横径<5%的发生率分别为71.93%、80.70%、89.47%、77.19%以及91.23%,明显多于闭合性脊柱裂胎儿,且差异有统计学意义(χ~2=18.997、30.334、57.799、33.255、41.137,P<0.05)。结论超声诊断胎儿脊柱裂具有重要的临床价值,尤其是对于开放性脊柱裂和闭合性脊柱裂胎儿的鉴别中具有很好的实用性。
Objective To observe the value and practical effect of ultrasound in diagnosis of fetal spina bifida and differential diagnosis of spinal bifida aperta and closed spina bifida. Methods 84 cases of children with spina bifida diagnosed in our hospital from January 2015 to June 2017 were selected and all patients were examined by ultrasound, and84 cases were divided into the open spina bifida group and closed spina bifida group, and the ratios of children combined with Arnold-Chiari Ⅱ type abnormalities of the two groups was observed, and the BPD, HC, cisterna magna disappearance, ventriculomegaly and cerebellar length were compared between the two groups. Results Of 84 cases of fetuses with spina bifida, there were 57 cases of children with open spina bifida, 52 cases combined with Arnold-Chiari Ⅱ type abnormalities, 27 cases of fetuses with closed spina bifida and no one with Arnold-Chiari Ⅱ type abnormalities,and the incidence rates of BDP〈-2 SD,HC〈-2 SD, cisterna magna disappearance, bilateral ventricle expansion, and cerebellar transverse diameter 5% of fetuses with open spina bifida were respectively 71.93%, 80.70%, 89.47%, 77.19%,91.23%, which were obviously more than those of fetuses with closed spina bifida, and the differences were statistically significant(χ^2=18.997, 30.334, 57.799, 33.255, 41.137,P〈0.05). Conclusion The clinical value of ultrasound in diagnosis of fetal spina bifida is very important, especially the practicability of ultrasound in differential diagnosis of open spina bifida and closed spina bifida is strong.
