摘要
目的探讨超声乳化白内障吸除术(PHACO)联合人工晶状体(IOL)植入术对糖尿病性白内障(DC)患者角膜内皮的影响。方法选择白内障患者163例,其中DC患者74例(糖尿病组)、单纯老年性白内障患者89例(对照组),所有患者均行PHACO联合IOL植入术。于术前及术后1个月、3个月应用角膜内皮显微镜检测两组患者的中央角膜内皮细胞密度(CCD)、变异系数(CV)、六角形细胞比例(6A%)、平均内皮细胞面积(AVE)和中央角膜厚度(CCT)。结果术后各时间点,糖尿病组的CCD、6A%均低于对照组,而术后1个月AVE高于对照组(均P<0.05)。两组患者的CCD、6A%均呈先降低后升高的变化趁势,而AVE呈先升高后降低的趋势(均P<0.05)。结论 PHACO联合IOL植入术对角膜内皮细胞有一定的损伤,DC患者比单纯老年性白内障患者角膜内皮细胞损伤更重,但随着时间推移,角膜内皮细胞的损伤有减轻趋势。
Objective To investigate the effect of phacoemulsification( PHACO) combined with intraocular lens( IOL) implantation on the corneal endothelium in patients with diabetic cataract( DC). Methods One hundred and sixty-three patients with cataract were enrolled,including 74 cases of DC( diabetic group) and 89 cases of simple senile cataract( control group),and all patients underwent PHACO combined with IOL implantation. Corneal endothelial microscope was used to detect the central corneal density( CCD) of endothelial cell,coefficient of variation( CV),percentage of hexagonal cells( 6 A%),average area of endothelial cell( AVE),and central corneal thickness in the two groups before and after one and three months of operation. Results At each time point after operation,both CCD and 6 A% in the diabetic group were lower than those in the control group,after one month of operation,AVE in the diabetic group was higher than that in the control group( all P〈 0. 05). In both groups,CCD and 6 A% decreased first and then increased,while AVE increased first and then decreased( all P〈 0. 05). Conclusion PHACO combined with IOL implantation has damage to corneal endothelial cells to a certain extent. Patients with DC suffer from severer damage to corneal endothelial cells compared to patients with simple senile cataract. However,the damage to corneal endothelial cells presents a tendency to reduce over time.
