摘要
目的对肝细胞肝癌(HCC)的多层螺旋CT(MSCT)征象与肿瘤血管生成指标的相关性进行探讨。方法选取经病理学证实的45例HCC标本作为研究对象,采用GE 64层螺旋CT对患者进行MSCT平扫和增强扫描,采用免疫组织化学法检测缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)的表达以及肿瘤微血管密度(MVD),然后对比分析其相关性。结果 45例HCC组织中,HIF-1α与VEGF的表达呈正相关(r_s=0.652,P=0.000);HIF-1α表达与MVD呈正相关(r_s=0.718,P=0.000);VEGF表达与MVD呈正相关(r_s=0.816,P=0.000)。HIF-1α及VEGF阳性表达患者的MVD值分别明显高于HIF-1α及VEGF阴性表达患者,差异有统计学意义(P<0.01)。不同肿瘤直径患者HIF-1α、VEGF的阳性表达率和MVD值比较,差异均无统计学意义(P>0.05);假包膜欠完整或无假包膜、侵袭转移高危患者HIF-1α、VEGF阳性表达率和MVD值均明显高于完整假包膜、侵袭转移低危患者(P<0.05);MSCT增强扫描动脉供血型患者HIF-1α阳性表达率低于双重供血型及少供血型患者(P<0.05),动脉供血型、双重供血型与少供血型患者VEGF阳性表达率和MVD值比较,差异均有统计学意义(P<0.05)。结论 HCC的MSCT征象与HIF-1α、VEGF和MVD值密切相关,可以作为判断HCC侵袭、血管生成和患者预后的依据。
Objective To explore the correlation between multi-slice computed tomography(MSCT) features and angiogenesis index in hepatocellular carcinoma(HCC). Method 45 cases of pathologically confirmed HCC specimens were included in the analysis, GE 64 slice spiral CT was utilized to perform MSCT plain scan and enhanced scan for these patients, immunohistochemical assay was used to detect hypoxia inducible factor-1 alpha(HIF-1α), the expression of vascular endothelial growth factor(VEGF) and tumor microvascular density(MVD), and then the correlation among these factors were analyzed. Result In 45 HCC tissues, HIF-1α was positively correlated with VEGF expression(rs=0.652, P=0.000); HIF-1α expression was positively correlated with MVD(rs=0.718, P=0.000); VEGF expression was positively correlated with MVD(rs=0.816, P=0.000). The MVD value in patients with positive expression of HIF-1α and VEGF were both significantly higher than in those with negative HIF-1α and VEGF, and the difference was statistically significant(P〈0.01). There was no significant difference between the positive expression rate of HIF-1α、VEGF and the MVD value in patients with dissimilar tumor diameter(P〉0.05); incomplete pseudocapsule or lack of pseudocapsule and with high risk of metastatic invasion were associated with higher positive expression rate of HIF-1α and VEGF as well as higher MVD value than those with intact pseudocapsule and low risk of metastatic invasion(P〈0.05); MSCT enhanced scanning as arterial blood supply indicated lower HIF-1α positive expression rate than those with dual blood supply or decreased blood supply(P〈0.05), furthermore, the VEGF positive expression rate and MVD value showed statistically significant difference among patients with arterial, dual or decreased blood supply(P〈0.05). Conclusion The MSCT features of HCC tissue are closely related to HIF-1α, VEGF expression and the MVD value, it may be applied as makers for invasion of HCC, angiogenesis and prognosis.
