Research progress of the clinical application of microwave ablation in the treatment of bone tumors

摘要 骨肿瘤是来源于骨或其附属组织的肿瘤,有良、恶性之分,其中恶性又分为原发性和继发性两类。良性骨肿瘤容易根治,预后好,恶性骨肿瘤容易局部复发和远处转移,预后差。骨肿瘤的治疗以外科切除治疗为主导,辅以放疗、化疗、靶向治疗、生物治疗等内科治疗手段^([1-3])。原发性恶性骨肿瘤手术治疗原则为肿瘤的完整切除、硬组织的重建和软组织的覆盖修复. The treatment of bone tumors is dominated by surgical resection and supplemented by radiotherapy, chemotherapy, targeted therapy, and biological therapy. Microwave ablation is a local thermal ablation technique. It is based on the basic feature that tumor cells are more sensitive to heat than normal cells. It has the features of high thermal efficiency, rapid temperature increase, complete necrosis in the coagulation zone, etc. It has been widely used in liver cancer, lung cancer and other fields. In the 1990 s, Fan Qingyu et al proposed the microwave ablation technique combined with surgical resection for the treatment of bone tumors, which achieves good results. In recent years, microwave ablation of bone tumors develops well. It has the same clinical effects as radiofrequency ablation in the treatment of osteoid osteoma. Microwave ablation treatment of giant cell tumors of the bone has the advantages of low local recurrence rate, good postoperative joint function and patient acceptance. Microwave ablation treatment of malignant bone tumors adjacent to the knee should be cautious, which may face the shortcomings of high postoperative recurrence rate and high fracture rate. It shows good efficacy for malignant bone tumors distal to the humerus. It has unique advantages in the treatment of pelvic malignant tumors, which simplifies the operation scale and has low postoperative complications, low recurrence rate, and high rate of good performance of limb joints. Microwave ablation of spine metastases has significant clinical effects. Compared with radiofrequency ablation, microwave ablation has high frequency, strong penetrating power, high temperature, multi-needle joint synergistic ablation, large ablation range, better efficacy, and superiority to high-resistance bone tumors. This article reviews recent advances in clinical applications.
