摘要
随着医学模式逐渐向生理—心理—社会模式的转变,卒中后抑郁的治疗日益受到关注。目前临床上使用的西药虽有一定的抗抑郁疗效,但有头晕、头痛、恶心、呕吐、纳差、失眠、肥胖、诱发癫痫等副作用。近年来,随着中医药事业的发展,中药治疗卒中后抑郁体现出独特优势,并逐渐走向多样化和综合化。高敏教授认为卒中后抑郁以肝郁为基础,肝郁气滞,脾失运化,心神失养发为郁病,其治疗重点在于疏肝解郁,兼以健脾益气、清肝泻火、理气化痰、养血安神、消食导滞等辨证治疗。通过长期临床实践及验证,运用以中药为基础,配合心理疏导的综合治疗模式,可显著提高卒中后抑郁的临床疗效。
Along with the medical mode transformed as the physiology-psychology-society mode,the attention is drawn on the treatment of post-stroke depression. Currently,the western medicines have a certain effect of anti-depression,but they induce some side effects such as dizziness,headache,nausea,vomiting,poor appetite,insomnia,obesity and induced-epilepsy. In recent years,TCM displays its unique advantages on post-stroke depression and its treatment is getting multiple and comprehensive. Prof. Gao Min believes that the liver qi stagnation is the basic pathogenesis of post-stroke depression. Liver qi stagnation results in dysfunction of spleen in transportation and transformation and malnutrition of heart mind. Finally,depression occurs. The treatment focuses on smoothing liver qi,associated with strengthening and benefiting spleen qi,reducing liver fire,regulating qi and resolving phlegm,nourishing blood and calming down the mind and promoting digestion. The treatment is given in compliance with the symptom differentiation. It is found through the long-term clinical practice and verification that Chinese herbal medicines improve significantly the clinical therapeutic effects on post-stroke depression in combination with comprehensive psychotherapy.
