摘要
目的探讨超敏C-反应蛋白(hs-CRP)与血清降钙素原(PCT)在新生儿感染性疾病中的表达与临床意义。方法选取2015年2月至2017年3月176例在淮北市人民医院新生儿科住院的新生儿,将其分为细菌感染组、非细菌感染组和健康对照组,同时进行hs-CRP和PCT检测和血培养,对检测结果进行分析比较。结果与对照组相比,细菌感染组与非细菌感染组新生儿hs-CRP和PCT水平均明显升高,差异均有统计学意义(F值分别为6.36、7.89,均P<0.01);与非细菌感染组比较,细菌感染组新生儿hs-CRP和PCT水平均明显升高,差异均有统计学意义(t值分别为4.26、5.13,均P<0.01);PCT检测特异度高于hsCRP,差异均有统计学意义(χ~2=7.12、6.99,P<0.05);而hs-CRP和PCT联合检测的敏感度与特异度较两者单独检测明显提高,差异均有统计学意义(χ~2值分别为6.92、6.36、7.25、6.56,均P<0.05)。结论 hs-CRP和PCT检测可尽早诊断和鉴别新生儿感染的临床表现,帮助合理使用抗菌药物。
Objective To investigate the expressions and the clinical significance of high sensitive C reactive protein (hs-CRP) and procalcitonin (PCT) in neonatal infectious diseases. Methods One hundred and seventy-six neonates hospitalized in Huaibei People's Hospital were divided into bacterium infection group, non bacterium infection group, and healthy control group. Serum levels of hs-CRP and PCT were detected and blood was cultured in three groups for analysis. Results Serum levels of hs-CRP and PCT of non bacterium infection group and bacterium infection group were significantly higher than those of the control group (F value was 6.36 and 7.89, respectively, both P〈0.01), meanwhile serum levels of hs-CRP and PCT of bacterium infection group increased significantly compared with those of non bacterium infection group (t value was 4.26 and 5.13, respectively, both P〈0. 01). The specificity of serum PCT detection was higher than that of hs-CRP (χ2 = 7.12 and 6.99, respectively, both P〈0.05). The sensitivity and specificity of detecting serum hs-CRP level combined with serum PCT level were remarkably higher than those of single test of hs-CRP or PCT (χ2 value was 6.92, 6.36, 7.25 and 6.56, respectively, all P〈0.05). Conclusion The detection of hs- CRP and PCT can diagnose and identify the clinical manifestations of neonatal infectious diseases as early as possible, and help rational use of antimicrobial agents.
