摘要
目的观察自发热包隔药灸对小儿慢性腹泻病患者分泌型免疫球蛋白A及细胞免疫水平的影响,评价其临床疗效。方法将48例慢性腹泻病患儿随机分为隔药灸组和对照组,隔药灸组采用自发热包隔药灸治疗,对照组口服蒙脱石滴剂及枯草杆菌二联活菌颗粒治疗,2组患儿均暂停添加新辅食,在病情需要时给予维持电解质平衡、补液等支持治疗。观察比较按疗程治疗后2组患儿的分泌型免疫球蛋白A和细胞免疫指标水平,并评价自发热包隔药灸治疗小儿慢性腹泻病的临床疗效。结果隔药灸组治愈率50%、总有效率91.67%;对照组治愈率37.5%,隔药灸组治愈率优于对照组(P<0.05)。2组疗程结束后隔药灸组分泌型免疫球蛋白A水平低于对照组;CD3+水平高于对照组;CD4+/CD8+比值大于对照组(均P<0.05)。结论自发热包隔药灸能较快控制小儿慢性腹泻患者的临床症状,对患儿分泌型免疫球蛋白A和细胞免疫指标水平的改善明显。该疗法确切有效,同时具有可操作性强,安全性高的优点,易于对小儿慢性腹泻患者治疗操作。
Objective To observe the effect of self-heating package partitioned moxibustion on secretory immunoglobulin A and cellular immunity in children with chronic diarrhea, and to evaluate its clinical effect. Methods 48 children with chronic diarrhea were randomly divided into partitioned moxibustion group and control group. The partitioned moxibustion group was treated with selfheating package partitioned moxibustion. The control group took oral drops of montmorillonite and live combined Bacillus subtilis two bacteria granule treatment. Two groups of children were suspended to add new foods to maintain electrolyte balance, rehydration support in the treatment of the condition needed. According to the observation after treatment, the secretory immunoglobulin A（Sig A） and cellular immunity level, the difference between the two groups of children with Sig A and cellular immunity index were observed, and the clinical curative effect of self-heating package partitioned moxibustion was compared. Results The cure rate of the partitioned moxibustion group was 50%, the total effective rate of the partitioned moxibustion group was 91.67%, the cure rate of the control group was 37.5%, and the cure rate of the of the partitioned moxibustion group was better than that of the control group（P〈0.05）. After treatment, the level of Sig A in the partitioned moxibustion group was lower than that of the control group, and the level of CD3 ＋of the partitioned moxibustion group was higher than that of the control group; and the ratio of CD4 ＋/CD8 ＋the partitioned moxibustion group was greater than that of the control group（ all P〈0.05）. Conclusion The self-heating package partitioned moxibustion can quickly control the clinical symptoms, and significantly improve immunoglobulin A and cellular immunity level. The therapy is accurate and effective, and has the advantages of strong maneuverability and high safety. It is easy to treat the patients with chronic diarrhea.
