摘要
目的探讨高血压患者胫前动脉血管舒张功能与红细胞分布宽度(RDW)的关系。方法选择2011年11月至2016年1月福建医科大学附属第一医院(我院)就诊的原发性高血压患者537(男277、女260)例,年龄(58.8±12.6)岁。选择同期未服用任何药物在我院门诊初诊的体检正常的非高血压患者100名作为对照组。采用高分辨超声测定反应性充血前后胫前动脉内径的变化率为内皮依赖性血管舒张(EDD)功能,含服硝酸甘油前后靶动脉内径的变化率为非内皮依赖性血管舒张(EID)功能。采用全自动血细胞分析仪测定红细胞数、RDW。结果高血压患者与正常对照者之间RDW无明显差异。随着年龄增加,高血压患者的RDW有升高趋势,胫前动脉EDD及EID均呈现下降趋势(均P<0.05)。相关分析显示,高血压患者中RDW与胫前动脉EDD无相关,与胫前动脉EID呈弱负相关(r=-0.094,P<0.05);多元线性回归分析显示:RDW是胫前动脉EID的影响因素,但影响很小(B=-0.005,P<0.05)。结论原发性高血压患者血RDW随年龄增大而增加,与胫前动脉EDD无关,与胫前动脉EID弱相关。
Objective To investigate the association between red blood cell distribution width (RDW) and vasodilatation in patients with essential hypertension. Methods A total of 537 hypertensive patients aged (58.8±12.6) years, including 277 males and 260 females, were enrolled. One hundred subjects without clinical evidence of by pertension were recruited as the normotensive control. The dilatation of anterior tibial artery induced by reactive hyperemia and nitroglycerin was examined by high-resolution vascular ultrasound, and was then defined as endotheli urn-dependent dilatation (EDD) and endothelium independent dilatation (EID). RDW was detected by automatic hemocyte analyzer. Results No difference in RDW was found between hypertensive patients and normotensives. EDD and EID reduced gradually with the increase of age in hypertensives. RDW exhibited an increasing trend with age in hypertensives. There was no correlation between EDD and RDW in patients with essential hypertension. There was weak correlation between EID and RDW in patients with essential hypertension (r=- 0. 094, P〈0.05). Multivariate linear regression analysis revealed that RDW was associated with EID ( B = - 0. 005, P〈 0.05 ). Conclusion RDW exhibited an increasing trend with age in hypertensives. RDW was not correlated with EDD, but was weak correlated with EID in patients with essential hypertension.
