摘要
目的探讨经纤维支气管镜联合局部化疗对中央型肺癌患者血清学指标的影响。方法将中央型肺癌患者82例按照入院时间顺序分为对照组和观察组,各41例。给予所有患者全身化疗,观察组在此基础上行纤维支气管镜局部化疗。观察2组治疗1个疗程后的临床疗效以及治疗前后血清学指标水平。结果观察组的治疗总有效率为63.41%,高于对照组的31.71%(P<0.05)。治疗前,2组的癌胚抗原(CEA)、细胞角蛋白片段(Cyfra21-1)、神经元特异性烯醇化酶(NSE)和糖链抗原19-9(CA19-9)水平均不具有统计学差异(P>0.05);治疗后,2组的CEA、Cyfra21-1、NSE和CA19-9水平均低于治疗前(P<0.05),且观察组治疗后的CEA、Cyfra21-1、NSE和CA19-9水平均低于对照组(P<0.05)。治疗前,2组的CD_4^+、CD_8^+和CD_4^+/CD_8^+水平均不具有统计学差异(P>0.05);治疗后,2组的CD_4^+和CD_4^+/CD_8^+水平高于治疗前(P<0.05),观察组CD_8^+水平低于治疗前(P<0.05),对照组的CD_8^+水平略低于治疗前但不具统计学差异(P>0.05);观察组治疗后的CD_4^+和CD_4^+/CD_8^+水平高于对照组且CD_8^+水平低于对照组(P<0.05)。治疗前,2组的基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)水平均不具有统计学差异(P>0.05);治疗后,2组的MMP-2和MMP-9水平低于治疗前(P<0.05);观察组治疗后的MMP-2和MMP-9水平低于对照组(P<0.05)。结论全身化疗基础上经纤维支气管镜联合局部化疗治疗中央型肺癌较单纯全身化疗治疗总有效率高,在降低血清肿瘤标志物和基质金属蛋白酶(MMP)水平以及改善患者全身免疫功能状态等方面更具优势。
Objective To discuss the effects of fiberoptic bronchoscopy combined with local chemotherapy on serological indexes of central lung cancer patients. Methods A total of 82 central lung cancer patients were studied and were divided into the control group( 41 cases) and the observation group( 41 cases) according to the time of admission. Both groups were given systemic chemotherapy and the observation group was given fiberoptic bronchoscopy combined with local chemotherapy additionally. The clinical efficacy of the 2 groups after 1 course of treatment were observed and serological indexes were observed before and after treatment. Results The total effective rate of the observation group was higher that that of the control group( P 〈 0. 05);Before treatment,cancer embryo antigen( CEA),cell keratin fragment( cyfra21-1),neuronal specific enolase( NSE) and sugar chain antigen 19-9( CA19-9) levels of the 2 groups had no statistical difference( P 〉 0. 05); CEA,cyfra21-1,NSE and CA19-9 levels of both groups after treatment were lower than those before treatment( P 〈 0. 05); After treatment,CEA,cyfra21-1,NSE and CA19-9 levels of the observation group were lower than those of the control group( P 〈 0. 05); Before treatment,CD4+,CD8+ and CD4+/CD8+ levels of the 2 groups had no statistical difference( P 〉 0. 05); CD4+ and CD4+/CD8+ levels of the 2 groups were higher and CD8+ level after treatment was lower than those before treatment( P 〈 0. 05); After treatment,CD4+ and CD4+/CD8+ levels of the observation group were higher and CD8+ level after treatment was lower than those of the control group( P 〈 0. 05);Before treatment,matrix metalloproteinase-2( MMP-2) and matrix metalloproteinase-9( MMP-9) levels of the 2 groups had no statistical difference( P 〉 0. 05); MMP-2 and MMP-9 levels of the 2 groups after treatment were lower than those before treatment( P 〈 0. 05); After treatment,MMP-2 and MMP-9 of the observation group were lower than those of the control group( P 〈 0. 05).Conclusion Fiberoptic bronchoscopy combined with local chemotherapy based on systemic chemotherapy show higher treatment total effective rate than single systemic chemotherapy. And it have more advantages in reducing serum tumor markers and matrix metalloproteinase( MMP) levels and improving systemic immune function.
