摘要
目的评价醒脑解郁汤联合推拿治疗卒中后抑郁(post stroke depression,PSD)的临床效果。方法连续选择2016年10月—2017年10月住院的PSD患者共100例,随机分为对照组和观察组,每组各50例。对照组采用西药盐酸氟西汀胶囊,观察组采用醒脑解郁汤,2组均联合推拿治疗,21 d为1疗程,共1疗程;比较2组患者治疗前后神经功能缺损的美国国立卫生院神经功能缺损评分(national institutes of health stroke scale,NIHSS)和抑郁汉密尔顿抑郁量表评分(Hamilton depression scale,HAMD),生活质量改良Barthel Index(modify Barthel Index,MBI)评分,总有效率和药物不良反应,血清5-羟色胺(5-Hydroxytryptamine,5-HT)、白细胞介素-6(interleukin-6,IL-6)和肿瘤坏死因子(tumor necrosis factor alpha,TNF-?)水平。结果治疗前,2组患者的NIHSS、HAMD、MBI、5-HT、IL-6和TNF-?均有可比性(P>0.05);与治疗前相比,治疗后2组患者的IHSS、HAMD、5-HT、IL-6和TNF-?均明显降低(P<0.05),MBI评分明显增加(P<0.05);与对照组比,观察组的IHSS、HAMD、5-HT、IL-6和TNF-?均明显降低(P<0.05),MBI评分增加(P<0.05)。2组均无严重不良反应(P>0.05)。结论醒脑解郁汤联合推拿治疗PSD有较好的近期安全性和有效性。
OBJECTIVE To study effect of Xingnao Jieyu prescription combined with massage therapy for post stroke depression（PSD）. METHODS A total of 100 consecutive PSD inpatients from October 2016 to October 2017 were enrolled and divided randomly into the control group and the observation group, with 50 cases in each group. The patients in the control group received fluoxetine hydrochloride capsules combined with massage therapy and the observation group adopted Xingnao Jieyu prescription combined with massage therapy for 21 d as 1 period. Compare national institutes of health stroke scale（NIHSS）, Hamilton depression scale（HAMD）, modify Barthel Index（MBI）, effect, side effect, serum 5-Hydroxytryptamine（5-HT）, interleukin-6（IL-6） and tumor necrosis factor alpha（TNF-α）. RESULTS Before treatment, the NIHSS, HAMD, MBI, 5-HT, IL-6 and TNF-α in 2 groups were comparable（P〉0.05）. Compared with the before treatment, the NIHSS, HAMD, 5-HT, IL-6 and TNF-α were decreased（P〈0.05）, MBI was increased in 2 groups（P〈0.05）. Compared with the control group, the NIHSS, HAMD, 5-HT, IL-6 and TNF-α were decreased（P〈0.05）, MBI had increased in the observation group（P〈0.05）. The effective cases and the total efficacy rate in the observation group were more（P〈0.05）. There were no severe adverse drug reactions in 2 groups. CONCLUSION It is better short-term safety and effectiveness of Xingnao Jieyu prescription combined with massage therapy for PSD patients.
