摘要
目的分析二、三年级小学生龋齿患病情况及其影响因素,为制定相关干预措施提供依据。方法分层整群抽取广州市4所小学二、三年级学生1 116名为研究对象,检查其患龋情况,并对学生及其家长进行问卷调查。结果小学生患龋率为72.3%(807/1 116),其中广州户籍儿童患龋率为52.2%(239/458),非广州户籍儿童为86.3%(568/658),差异有统计学意义(χ~2=157.19,P<0.01);二年级学生患龋率(77.0%)高于三年级(67.7%)(χ~2=12.21,P<0.01)。单因素逐步Logistic回归分析显示,户籍、年级、喝碳酸饮料、食用甜点巧克力、开始刷牙年龄、刷牙次数、刷牙方法、是否使用含氟牙膏、照顾孩子的人、居住环境、医疗来源、父母学历、父母职业、家庭经济收入14个因素均入选方程;多因素Logistic回归分析显示,户籍、喝碳酸饮料、是否使用含氟牙膏及父母学历等均为龋齿发生的独立影响因素(P值均<0.01)。结论二、三年级小学生是龋病发生的高危群体,龋齿发生与社会生活环境、饮食习惯、口腔卫生习惯等因素关系密切。
Objective To characterize dental caries among primary school students in Guangzhou. Methods By using stratified cluster sampling, 1 116 grade 2 and 3 students from four primary schools were chosen. Oral examination and a parent questionnaire survey was conducted. Results There were 569 boys and 547 girls in this study. The average age was（ 7.91±0.02）years. About 72.3%（ 807） students had dental caries, among them 52. 2% were local resident. The prevalence was significantly higher（ 86.3%, 568/658） among non-resident children（ χ^2= 157.19, P〈0.01）. Prevalence of dental caries was higher among grade2 than grade 3 students（ χ^2= 12.21, P〈0.01）; Multi-factor stepwise Logistic regression showed that local residency, grade, beverages, candy chocolate, age and number of tooth brushing, tooth brushing method, fluoride toothpaste usage, caregiver, living environment, parents＇ education and occupation, as well as household income were included in the equation, while local residency,beverage drinking, fluoride toothpaste usage and parental education closely associated with dental caries（ P〈0.01）. Conclusion Low grade elementary students are especially vulnerable to dental caries, which is associated with living environment, dietary and oral hygiene habits of children.
