摘要
目的:探讨"三明治"固定法在ICU中心静脉导管(CVC)患者中的应用效果。方法:将660例需留置CVC患者按照入组先后顺序采用数字表法分为对照组A、对照组B及观察组各220例,对照组A采用透明敷料对准穿刺点中心固定,对照组B采用缝线加纱布固定,观察组采用"三明治"固定法,比较三组CVC导管并发症、中心静脉导管相关性血流感染(CLABSI)发生率及满意度。结果:观察组皮肤损伤或感染发生率显著低于对照组A、B(P<0.05);观察组CLABSI发生率显著低于对照组A(P<0.05);观察组对CVC导管固定的舒适度、日常生活影响及疼痛评价得分均显著高于对照组A、B(P<0.05);对照组B对CVC导管固定的舒适度及疼痛评价得分均显著高于对照组A(P<0.05)。结论:"三明治"固定法与常法固定比较,可降低皮肤损伤或感染、CLABSI发生率,提升CVC导管留置患者的满意度。
Objective： To explore the effect of sandwich fixation in patients with ICU central venous catheter. Methods： A total of 660 patients who had to be retained CVC were divided into the control group A,the control group B and the observation group,220 cases in each group. The control group A was fixed with the transparent dressing at the center of the puncture point,the control group B was fixed with the suture plus the gauze,the observation group adopted the sandwich fixation method. The complications of the three groups of CVC catheters and the incidence and satisfaction of central venous catheter-related bloodstream infection（ CLABSI） were compared. Results： The incidence of skin injury or infection was significantly lower in the observation group than in the control group A、B（ P〈0. 05）; the incidence of CLABSI in the observation group was significantly lower than that in the control group A（ P〈0. 05）; The score of the comfort degree,daily life influence and pain evaluation of the CVC catheter in the observation group were significantly higher than that of the control group A、B（ P〈0. 05）,and the score of the comfort degree and pain evaluation of the CVC catheter in the control group B was significantly higher than that of the control group A（ P〈0. 05）. Conclusion： Compared with conventional fixation,The＂sandwich＂fixation method can reduce the skin damage or infection,the incidence of CLABSI,and improve the satisfaction of patients with CVC catheter indwelling.
