摘要
目的探讨硫辛酸配合鞘内注射地塞米松对中枢神经系统脱髓鞘疾病患者神经功能的影响。方法选取2013-01—2016-02就诊于南阳市中心医院的70例中枢神经系统脱髓鞘疾病患者,按照随机数字表法分为研究组和对照组各35例,对照组在常规治疗基础上给予地塞米松鞘内注射,研究组在对照组基础上另给予硫辛酸治疗。观察并比较2组患者的治疗效果、神经功能缺损程度改善情况、脑脊液中蛋白含量变化及不良反应情况。结果研究组治疗总有效率为97.14%显著高于对照组的77.14%(P<0.05);2组患者治疗后1个月、3个月神经功能缺损程度评分显著低于治疗前,且研究组患者神经功能缺损程度评分显著低于对照组(P<0.05);研究组治疗后脑脊液中蛋白含量及白细胞恢复正常时间显著短于对照组,且研究组在治疗后第2周起脑脊液中蛋白含量及白细胞升高者显著减少,第4周起降为0,对照组自第3周起脑脊液中蛋白含量及白细胞升高者显著减少,第5周仍然存在脑脊液中蛋白含量及白细胞升高者(P<0.05);2组均无不良反应发生。结论采用硫辛酸配合鞘内注射地塞米松治疗能够促进患者神经功能快速恢复,具有较高安全性和良好治疗效果。
To discuss the effect of thioctic acid combined with intrathecal injection of dexamethasone on neurologic function in patients with demyelinating diseases of the central nervous system.Methods Seventy cases of patients with demyelinating diseases of the central nervous system treated in our hospital from January 2013 to February 2016 were slected and divided into study group and control group according to the random digital table method,with thirty-five cases in each.The control group were treated with intrathecal injection of dexamethasone based on the conventional therapy,while the study group were treated with thioctic acid on the basis of the control group.The treatment effect,the improvement of degrees of neural function defect and changes of protein content in cerebrospinal fluid and adverse reactions in the two groups were observed and compared.Results The total treatment effective rate in the study group was 97.14%,significantly higher than that of 77.14% in the control group(P〈0.05).The scores of degrees of neural function defect in the two groups after 1 month and 3 months of treatment were significantly lower than those before tretment,which in the study group were significantly lower than those in the control group(P 〈0.05).The recovery time of protein content in the cerebrospinal fluid and white blood cells in the study group was significantly shorter than that in the control group,and the number of patients whose protein content in the cerebrospinal fluid and white blood cells were increased was significantly decreased since the second week,which was decreased to 0 since the forth week,while the number in the control group were significant decreased since the third week,and there were still patients whose protein content in the cerebrospinal fluid and white blood cells were increased at the fifth week(P 〈0.05).Conclusion The application of thioctic acid combined with intrathecal injection of dexamethasone can promote the neurological function in patients with demyelinating diseases of the central nervous system,It can promote rapid recovery with high safety and good effect.
