骨标志物在绝经后骨质疏松女性中检测的意义

摘要 目的:研究骨标志物硬化蛋白(sclerostin,SOST)、核因子-κB受体活化因子配体(receptoractivator of NF-κB ligand,RANKL)、Ⅰ型前胶原氨基端延长肽(N-terminal propeptide of type 1 procollagen,P1NP)、N端中段骨钙素(N-MID Osteocalcin,N-MID-OT)等骨标志物在绝经女性骨质疏松症中的检测意义及价值。方法:收集我院2014年12月-2016年3月非绝经期正常健康体检女性40名(对照组),同期绝经后骨质疏松女性120名(实验组),分别检测以上各骨标志物指标在外周血中的水平,并根据是否运动和口服碳酸钙维生素D3随机将实验组平均分为3组:A:运动结合口服维生素D3;B:单一运动或者口服维生素D3;C:无运动且无口服维生素D3。分别检测实验组、对照组各检测指标的变化。结果:实验组中各检测指标与对照组相比差异具有统计学意义(P<0.05);实验组中RANKL、P1NP、N-MID-OT等指标变化为A<B<C,AB两组与运动和/或口服维生素D3之前比较差异具有统计学意义(P<0.05)。结论:SOST mRNA在绝经骨质疏松女性患者外周血中高表达,进行持续的人为干预可以对女性绝经后骨标志物的水平有一定的影响,并且能够降低骨量丢失和骨吸收的风险,有效预防绝经后女性骨质疏松症的发生。 Objective To study the detection value of bone markers such as SOST,RANKL,P1 NP and N-MID-OT in osteoporosis of postmenopausal women. Methods Forty non-menopausal healthy women( control group) and 120 patients with postmenopausal osteoporosis( experimental group) were enrolled in this study during the period of December 2014 to March 2016 in our hospital. The bone markers were detected in peripheral blood of all subjects. According to whether exercise and oral calcium carbonate vitamin D3,the experimental group was further divided into three sub groups: A,exercise combined with oral vitamin D3; B,single exercise or oral vitamin D3; C,no exercise and no oral vitamin D3. The changes of the test indexes were measured in the experimental group and the control group respectively. Results All indexes in the experimental group were statistically different from those in the control group( P 0. 05); the changes of RANKL,P1 NP and N-MID-OT in the experimental group showed a pattern of A〈B〈C. The difference between AB two group and exercise and/or oral vitamin D3 was statistically significant( P 〈 0.05). Conclusion SOST mRNA is highly expressed in peripheral blood of postmenopausal women with osteoporosis. Continuous human intervention may have a certain impact on the level of bone markers in postmenopausal women,and can reduce the risk of bone loss and bone absorption,and effectively prevent the occurrence of osteoporosis in postmenopausal women.
