摘要
目的探讨经皮脊柱内镜腰椎间盘髓核摘除术(percutaneous endoscopic lumbar discectomy,PELD)治疗腰椎间盘突出症后综合康复训练的疗效。方法 2016年7-10月在本院行PELD手术治疗的腰椎间盘突出症患者100例,按照随机数字法分配到对照组和试验组各50例,术后试验组进行综合康复训练(腰椎微创术后康复训练操),对照组给予术后常规干预,不进行特殊指导,并于术前和术后3 d、7 d、10 d、2周、3周、1个月、2个月、3个月共9次通过门诊和网络平台相结合的方式行VAS评估、Oswestry功能障碍指数问卷表(oswestry disability index score,ODI)调查和起坐行走测试(timed up and go,TUG)评估。结果两组术前腰部VAS评分、腿部VAS评分、ODI评分、TUG评分差异无统计学意义(P均>0.05)。术后3个月试验组各项指标均优于对照组[腰部VAS评分:(1.28±0.51)vs(1.83±0.84),P=0.006;腿部VAS评分:(1.36±0.71)vs(1.75±0.59),P=0.008;ODI评分:术后3个月:(13.28%±4.32%)vs(17.76%±4.67%),P=0.007。TUG评分:(7.25±2.30)s vs(9.00±2.96)s,P=0.010]。结论综合康复训练可有效提高经皮脊柱内镜腰椎间盘髓核摘除术治疗效果,缓解术后疼痛,促进患者恢复。
Objective To study the effect of comprehensive rehabilitation training on patients with lumbar disc herniation after percutaneous endoscopic lumbar discectomy （PELD）. Methods From July to October in 2016, 100 patients who underwent PELD for herniated lumbar disc in our hospital were randomly assigned to control group （n=50） and experimental group （n=50）. Patients in experimental group received comprehensive rehabilitation training and control group received postoperative routine rehabilitation. Visual analog score （VAS）, oswestry disability index score （ODI）, timed up and go （TUG） score of low back pain and leg pain were collected at 3 d, 7 d, 10 d, 2 weeks, 3 weeks, 1, 2 and 3 months after operation. Results There was no difference in low back pain, leg pain, ODI score, and TUG score between two groups （all P 〉 0.05）. At 3 months after training, all these indicators were superior in experimental group compared to control group [low back pain： （1.28 ± 0.51） vs （1.83 ± 0.84）; leg pain： （1.36 ± 0.71） vs （1.75 ± 0.59）; ODI： （13.28 ± 4.32） vs （17.76 ±4.67）; TUG： （7.25 ± 2.30） vs （9 ± 2.96）; all P 〈 0.05]. Conclusion Comprehensive rehabilitation training can improve the treatment effect of PELD on lumbar disc herniation, alleviate postoperative pain and promote patient＇s recovery.
