摘要
目的探讨血清胱抑素C(Cys C)、β_2微球蛋白(β_2-MG)在早期2型糖尿病肾病(DN)中的诊断价值。方法选取2016年10月至2017年10月间云浮市人民医院糖尿病患者140例,根据患者24 h尿微量白蛋白排泄率(UAER)将患者分为临床糖尿病肾病组29例、早期糖尿病肾病组42例和非糖尿病肾病组69例,同期健康体检者40例作为对照组,观察四组受检者Cys C、β_2-MG、尿素氮(BUN)的变化情况,并采用Pearson相关分析各指标间的相关性。结果临床糖尿病肾病组患者的Cys C、β_2-MG和BUN水平明显高于其他三组,早期糖尿病肾病组患者的Cys C、β_2-MG和BUN水平又明显高于非糖尿病肾病组和对照组,差异均有统计学意义(P<0.05);临床糖尿病肾病组患者的Cys C、β_2-MG和BUN的阳性检出率明显高于其他三组,早期糖尿病组患者的Cys C、β_2-MG检出阳性率明显高于非糖尿病组和正常对照组,差异均有统计学意义(P<0.05);糖尿病肾病患者的Cys C、β_2-MG检出阳性率明显高于BUN,差异均有统计学意义(P<0.05);Cys C与β_2-MG、BUN呈正相关(r=0.652、0.534,P<0.05);同样β_2-MG与Cys C、BUN呈正相关(r=0.652、0.485,P<0.05)。结论糖尿病肾病患者Cys C、β_2-MG水平均明显升高,且随着病变程度的加重呈升高趋势。在早期糖尿病肾病诊断中,Cys C、β_2-MG检出阳性率明显高于BUN,是一种良好的检测指标。
Objective To investigate the diagnostic value of serum cystatin C(Cys C) and β2 microglobulin(β2-MG) in early type 2 diabetic nephropathy. Methods A total of 140 patients with diabetes, who admitted to Yunfu People's Hospital from October 2016 to October 2017, were selected and divided into the clinical diabetic nephropathy group,early diabetic nephropathy group and non-diabetic nephropathy group according to the 24-hour UAER. In the same period, 40 cases of healthy physical examination were enrolled as the control group. The changes of Cys C, β2-MGM, blood urea nitrogen(BUN) were observed, and the correlation between the four groups were observed. Results Cys C, β2-MG,BUN in the clinical diabetic nephropathy group were significantly higher than those of the other three groups, and the Cys C, β2-MG, BUN in the early diabetic nephropathy group were significantly higher than those in the non-diabetic nephropathy group and the control group(P〈0.05). The positive rate of Cys C, β2-MG, BUN in the clinical diabetic nephropathy group was significantly higher than that of the other three groups, and the positive rate of Cys C, β2-MG in the early diabetic group was significantly higher than that of non-diabetic nephropathy group and control group(P〈0.05). Cys C was positively correlated with β2-MG and BUN(r=0.652, 0.534, P〈0.05); β2-MG was positively correlated with Cys C and BUN(r=0.652, 0.485, P〈0.05). Conclusion The levels of Cys C and β2-MG are significantly increased in patients with diabetic nephropathy, and they increase with the severity of the disease. In the early diagnosis of diabetic nephropathy,the positive rate of Cys C and β2-MG is significantly higher than that of BUN, which is a good detection index.
